摘 要:通过肝穿标本随机取材做透射电镜观察,石蜡标本定位取材分别做透射电镜及扫描电镜观察,X线能谱分析,研究了亚急性重症肝炎肝细胞内淤胆、坏死、再生的超微结构改变。详细描述了胆栓、肝细胞内胆色素的透射电镜图像及扫描电镜下呈钟乳石状的立体形态。探讨了肝细胞内淤胆导致胆汁凝固性坏死,淤积的胆色素凝块最终被胶原纤维包绕的发生发展过程。材料表明亚急性重症肝炎残存肝细胞岛边缘呈出芽状再生的假胆管与原胆道系统不相通,岛周过度增生的胶原纤维阻抑了肝细胞的再生。Autopsy liver specimens from 10 subacute severe hepatitis ( SSH ) Patients were taken at random; selected areas of the Paraffin 'block face' were then thin-sectioned and viewed by using transmission and scanning electron microscopies. The ultrastructural changes of cholestasis, necrosis and hePatocyte regeneration in SSH were studied. The Picture of bile Plugs and intrahePatoc-ytic bile Pigments, which appeared stalactite-like under SEM, was described. A Process was reconstructed to define how the intrahepatocytic cholestasis leads to chole-coagulative necrosis and the static bile Pigment clot is eventually surrounded by collagen fibers. The electron microscopy also showed that the regeneration of remnant hepatocyte islets was bud-like, with the newly formed Pseudoducts not communicating with the original biliary duct system, and thet the regeneration of hepatocyte islets was restricted by the hyperpfoliferated collagen fibers around them.
