房间隔缺损介入治疗后右房室重构研究

摘　　要：目的评价卡托普利及氯沙坦逆转房间隔缺损(ASD)封堵术后右房右室重构,改善心脏功能的临床疗效。方法 90例成功进行了封堵治疗的ASD患者,男40例,女50例;年龄14-78岁(32.12±10.83)岁。随机分为卡托普利组、氯沙坦组和对照组。术前、术后3d及术后6个月行超声心动图检查(TTE),观察右房室重构数据。结果与术前相比3组右房、右室内径及右室舒张末期容积均有减小,左室内径增大,左右心比例趋于正常;卡托普利组与氯沙坦组右房右室改变均较对照组有明显不同,具有统计学意义(P<0.05),而两组间改变无统计学意义(P>0.05)。结论卡托普利和氯沙坦均有助于逆转ASD所致的心脏病理变化,逆转右房右室重构,改善心功能。Objective To evaluate the clinical efficacy of Captopril and Losartan to reverse right atrium and right ventricle re-modeling after transcatheter closure of atrial septal defect.Methods Ninety patients after transcatheter closure of atrial septal defect were divided into 3 groups：Captopril group,Losartan group and control group.Ehocardiography examinations was performed before and 6 month after closure of ASD,To observe right atrium and right ventricular remodeling data.Results Comparing with captopril group,Losartan group and control group the right atrium,RAESd,RVDd,RVEDV were significantly decreased,while the left atrium and LA ESd,LVDd,LVEDV significantly increased（P〈0.05）,but there were no significant differences between captopril group and Losartan group（P〉0.05）.Conclusion Captopril and Losartan can reverse right atrium and right ventricle remodeling after transcatheter closure of atrial septal defect.
