欣普贝生应用于50例足月胎膜早破引产的临床观察

摘　　要：探讨欣普贝生用于足月妊娠胎膜早破促宫颈成熟及引产的有效性及安全性。方法:对90例胎膜早破、Bishop宫颈评分<6分、无妊娠合并症、单胎、头位并要求阴道试产的初产妇,选50例阴道放置欣普贝生促宫颈成熟作为研究组,另选40例同样条件的初产妇用催产素促宫颈成熟作为对照组。比较两组孕妇的Bishop评分变化、临产情况及分娩的发生时间、感染率、剖宫产率、对胎儿及新生儿的影响。结果:研究组孕妇给药后,24h后39例直接进入产程,其余7例Bishop评分提高≥2分。而对照组仅14例进入产程,9例Bishop评分提高≥2分,两组比较,差异有统计学意义(P<0.01)。研究组从给药到临产的时间平均为6h40min,对照组平均为11h15min,两组比较,差异有统计学意义(P<0.05)。研究组的剖宫产率为14%,而对照组则为35%。结论:欣普贝生可安全、有效地用于足月胎膜早破引产。Discussion efficacy and safety of the Dinoprostone Suppositories used for Full-term pregnancy,premature rupture of membranes cervical ripening and induction.Method：90 cases of premature rupture of membranes,Bishop cervical score 6 points,no pregnancy complications,single fetus,cephalic presentation and vaginal trial of labor required primipara.Selected 50 cases of vaginal Dinoprostone Suppositories cervical ripening as a research group,Replacement of the 40 cases with the same conditions of early maternal oxytocin for cervical ripening as a control group.Bishop score were compared changes in pregnant women,labor conditions and delivery time of occurrence,infection,cesarean section rate,the fetus and newborn.Results：Study group of pregnant women after administration,24 h after 39 cases directly into the production process,and the remaining 7 cases improved Bishop score ≥ 2 points.While the control group,only 14 cases into the production process,9 patients improved Bishop score ≥ 2 points,.Two groups,the difference was statistically significant（P 0.01）.Research group from the administration to give birth to an average of 6h40min,control group average of 11 h15 min,Two groups,the difference was statistically significant（P 0.05）.Study Group cesarean section rate was 14%,while the control group was 35%.Conclusion：Dinoprostone Suppositories can be safe and effective for the induction of labor at term premature rupture of membranes.
