肝移植术后早期白细胞分类计数与CD4^+T细胞ImmuKnow法ATP值的相关性研究

摘　　要：目的探讨临床常用的白细胞(WBC)分类计数与ImmuKnow法检测CD4+T细胞内的腺苷三磷酸(ATP)值的相关性,为临床提供一种快速判断细胞免疫功能的方法。方法选择50例行经典原位肝移植术受者术后1周至1个月内的样本,进行WBC分类计数并采用ImmuKnow法检测CD4+T细胞内ATP值,分析两者的相关性。随机选择5例肝移植受者于术后1、2、3、4、5周分别重复检测上述指标,进一步验证两者动态变化的相关性。结果 50例行肝移植受者ImmuKnow法ATP值为446.7±219.1μg/L,WBC总数为(8.08±3.62)×109/L,与ImmuKnow法ATP值的相关系数(r)为0.659(P<0.001);中性粒细胞计数为(6.07±3.07)×109/L,与ImmuKnow法ATP值的r为0.593(P<0.001);单核细胞计数为(0.53±0.25)×109/L,与ImmuKnow法ATP值的r为0.371(P=0.006)。淋巴细胞计数为(1.13±0.69)×109/L,与ImmuKnow ATP值无相关关系(r=0.138,P=0.135)。5例肝移植受者术后重复检测,WBC总数的变化与ImmuKnow法ATP值的变化呈正相关,r均>0.5,但由于样本量较少,仅有1例差异有统计学意义。结论肝移植术后早期WBC计数与CD4+T细胞ImmuKnow法ATP值具有一定的正相关性,WBC计数的变化可以在一定程度上反映ImmuKnow法ATP值的变化。Objective To investigate the correlation between blood white blood cell（WBC） count and adenosine triphosphate（ATP） value in CD4＋T cell by ImmuKnow method,so as to provide a method to rapidly identify cellular immune function.Methods A total of 50 recipients of classic orthotopic liver transplantation（OLT） were enrolled.The 50 samples of recipents from 1 week to 1 month after the transplanting operation were collected for the determination of relativity between WBC count and ATP value in CD4＋T cell by ImmuKnow method,and samples of 5 recipents selected randomly were collected for duplicated determination of the indices in Week 1,2,3,4 and 5 after the transplanting operation to further verify the correlation of their dynamic changes.Results WBC count had the high correlation with ATP value by ImmuKnow method at r=0.659（P0.001）,followed by neutrophile granulocyte count [（6.07±3.07）×109/L] at r=0.593（P0.001） and mononuclear cell count [（0.53±0.25）×109/L] at r=0.371（P=0.06）.Lymphocyte count[（1.13±0.69）×109/L] was insignificant for ATP value by ImmuKnow method（r=0.138,P=0.135）.The 5 recipents were repeatedly tested for ATP values by ImmuKnow method,which were positively correlated to WBC counts with r0.5.However,the number of samples was minimal,so only 1 sample was significant for statistics.Conclusions WBC count in early stage after the liver transplantion has some positive correlation with ATP value in CD4＋T cell by ImmuKnow method,and the changes of WBC count can reflect the changes of ATP value to some extent.
