尘螨、霉菌和花粉变应原结膜激发试验结果分析

摘　　要：目的通过对尘螨、霉菌及花粉变应原结膜激发试验(CPT)的结果分析,探讨变应原种类对结膜激发试验结果的影响。方法选择2008年至2009年符合变应性鼻炎、结膜炎和(或)支气管哮喘诊断的患者274例,根据变应原种类分为3组:尘螨组130例,霉菌组84例,花粉组60例。所有入选者均进行变应原结膜激发试验,采用北京协和医院眼结膜激发试验评估标准分别记录,同时进行血清特异性免疫球蛋白E(sIgE)检测。结果 3组变应原结膜激发试验结果阳性率在sIgE阴性的病例中分别为24.5%、7.9%、33.3%,两两比较其中尘螨组与霉菌组比较差异无统计学意义(P>0.05),而花粉组与尘螨组、霉菌组之间差异均有统计学意义(P<0.05);3组变应原结膜激发试验结果阳性率在sIgE阳性的病例中分别为70.0%、63.0%、81.0%,两两比较差异均无统计学意义(P>0.05),相关分析变应原结膜激发试验结果分级显示与变应原种类无明确相关(r=0.1323,P>0.05),而与sIgE级别相关(r=0.4657,P<0.05)。结论在sIgE阴性病例中,花粉变应原结膜激发试验较尘螨、霉菌敏感;而在sIgE阳性病例中,变应原结膜激发试验敏感性不受变应原种类影响,其分级与sIgE级别相关。Objective To investigate the effects of different allergens including house mite, fungi, pollen -allergen on the results of conjunctival provocation tests （CPT）. Methods Two hundred and seventy four allergic patients were enrolled in the study from 2008 to 2009, who were diagnosed as allergic rhinitis, conjunctivitis and （or） bronchial asthma,and the patients were divided into three groups：house mite group （ n = 130）, fungus group （ n = 84） and pollen group （ n = 60）. All the patients received CPT with different allergens, and the scores were recorded according to the CPT evaluation criterion instituted by Beijing Xiehe Hospital, and the levels of serum specific immuno globulin E （sIgE） were detected at the same time. Results Among the patients whose sIgE was negative,the positive rates of CPT with specific allergens were 24.5% in house mite group,7.9% in fungus group and 33.3% in pollen group respectively ,and there was no significant difference between house mite group and fungus group （ P 〉 0.05）, however, there was a significant difference between pollen group and house mite group or fungus group （ P 〈 0.05 ）. Among the patients whose sIgE was positive, the positive rates of CPT with specific allergens were 70.0% in house mite group, 63.0% in fungus group and 81. 0% in pollen group respectively, and there was no significant difference between the two groups each other（ P 〉 0.05 ）. The correlation analysis for the results of CPT showed that the grading was not correlated with the kinds of allergens（ r =0. 1323, P 〉 O. 05 ）, however, which was related with the grades of sIgE （ r = 0. 4657, P 〈 0.05 ）. Conclusion In the patients whose slgE is negative, the CPT with pollen - allergen is more sensitive than that with house mite or fungus,however,in the patients whose slgE is positive, the sensibility of CPT is not affected by the kinds of allergens, but its grading is related with the grades of sIgE.
