脉安舒颗粒对不稳定型心绞痛患者血清sCD40L的影响

摘　　要：目的观察脉安舒颗粒对不稳定型心绞痛(UAP)患者血清可溶性CD40配体(sCD40L)的影响。方法将60例气虚血瘀证UAP患者采用随机分层法分为治疗组30例和对照组30例。两组患者均按内科常规治疗,治疗组患者在常规治疗的基础上,加用脉安舒颗粒。治疗4周后,观察两组患者心绞痛发作频率、心电图及血清sCD40L的变化。结果治疗组心绞痛显效率、总有效率及心电图总有效率均明显优于对照组(P<0.01),同时治疗组血清sCD40L水平的下降幅度大于对照组(P<0.01)。结论脉安舒颗粒与常规西药合用在缓解心绞痛、改善心电图及降低血清sCD40L等方面均明显优于单用西药治疗。表明脉安舒颗粒治疗UAP有较好疗效,能显著降低血清sCD40L水平,抑制免疫炎症反应,从而减缓动脉粥样硬化(AS)斑块的进展,起到稳定斑块的作用。推测脉安舒颗粒通过降低血清sCD40L含量而稳定AS斑块,是其治疗UAP的有效机制之一。Objective To observe the impacts of maianshu granules on serum soluble CD40 ligand（sCD40L）for the patients with unstable angina pectoris（UAP）.Methods 60 UAP patients of qi deficiency and blood stagnation pattern were divided randomly in a treatment group（30 cases）and a control group（30 cases）.The conventional treatment of internal medicine was applied to either group.Additionally,maianshu granules were supplemented in treatment group.In 4 weeks treatment,the attack frequency of angina pectoris,ECG and sCD40L were observed in two groups.Results The remarkably effective rate,total effective rate and total effective rate of ECG in treatment group were all apparently superior to those in control group（P0.01）.Additionally,the down-regulation amplitude of serum sCD40L in treatment group was higher than that in control group（P0.01）.Conclusion The combined therapy with maianshu granules and conventional western medicine treatment relieves angina pectoris,improves ECG and down-regulates sCD40L,and its efficacies are superior apparently to those of simple western medicine treatment.It is shown that maianshu granules have a better efficacy on UAP,significantly reduces serum sCD40L level and inhibits immune inflammatory reaction so as to reduce the progression of atherosclerotic plagues and stabilize the plagues.It is deduced that maianshu granules stabilize atherosclerotic plagues through reducing serum sCD40L level,which is one of the effective mechanisms in treatment of UAP.
