祁阳县实施结核病防治规划终期(2001-2010年)卫生经济效应

摘　　要：目的从卫生经济学角度分析评价祁阳县结核病防治规划的经济学效应,探索该规划的合理性与收益性。方法收集2001-2010年结核病防治规划季报、年报及人口学资料,采用卫生经济学理论与方法对结核病防治规划的成本-效果、成本-效用及成本-效益进行分析评价。结果 10年间政府及相关国际组织投入经费254.3万元,每发现并治疗1例患者费用为338.71元(254.3万/7 508),病人投入费用为812.8万元,每例患者为1 082.6元(812.8万/7 508)。10年间共治愈病人3 715例,共挽回DALY39 152年,按祁阳县平均期望寿命73岁计算[4],共获得536(39 152/23)个完整生命。减少新肺结核病人5 573例,节省费用3 566.72万元。按国内生产总值(GDP计算),10年平均人均GDP7 259.4元/年,挽回GDP2.8亿元。按祁阳县农民人均纯收入3 752.9元计算,避免了因病造成的个人收入损失达14 693.4万元。结论 10年来祁阳县结核病防治规划的实施,各项指标达到了《湖南省结核病防治规划(2001-2010年)》的要求,取得了明显的社会效益和经济效益,为保障全县人民群众身体健康作出了积极贡献。Objective Based on the view of the health economics,we analyzed and evaluated the economic effects of tuberculosis control planning in Qiyang County,and then explored the feasibility and profitability of this planning.Methods Seasonal and annual repors of tuberculosis control planning and demographic data from 2001 to 20010 were collected.With the health-economic theory and methods,we evaluated the cost-effectiveness,cost-utility and cost-benefit of tuberculosis control planning.Results Government and related international organizations had been invested 2.543 million Yuan during the last 10 years.Each identified and treated patient spent about 338.71 Yuan（2.543 million/7,508）.The investment cost to patients was 8.128 million Yuan,and the cost of each patient was 1,082.6 Yuan（8.128 million/7,508）.A total of 3,715 patients had been cured during the last 10 years,and 39,152 DALYs（disability-adjusted life years） were retrieved.Totally 536 complete lives were obtained according to the average anticipated lifetime of 73 years in Qiyang County.5,573 new patients with pulmonary TB were reduced and 35.6672 million Yuan were saved.GDP per capita was 7,259.4 Yuan per year during the last 10 years,and 0.28 billion Yuan of GDP was retrieved.146.934 million Yuan of personal income loss which resulted from TB were avoided according to the rural per capita net income（3,752.9 Yuan） in Qiyang.Conclusions With the implementation of tuberculosis control planning,various indexes reached the demands of ＂The Planning of Tuberculosis Control in Hunan（2001~2010）＂,which obtained significantly social benefit and economic benefit,and made a contribution to ensuring the health of the people all over the county.
