撬拨复位与切开复位固定治疗跟骨关节内骨折  

Percutaneous Reduction and Fixation Versus Open Reduction and Fixation for Displaced Intra-articular Calcaneus Fractures

摘　　要：目的前瞻性比较经皮撬拨复位内固定与切开复位内固定治疗移位型跟骨关节内骨折的临床疗效。方法随机方法将2008年3月7日至2010年1月22日收治的40例移位型跟骨关节内骨折患者分为经皮撬拨复位内固定组和切开复位内固定组。结果术后第2天及术后1年,两组患者在B(o|¨)hler角、侧位片上跟骨长度、轴位片上跟骨宽度方面无明显差异(P>0.05);术后1年功能评分无明显差异(P>0.05);经皮撬拨复位组术后无一例出现切口感染,切开复位内固定组术后6例出现切口感染,两组比较存在统计学差异(P<0.05)。结论经皮撬拨复位内固定与切开复位内固定疗效相当。但经皮撬拔复位内固定可以降低术后皮肤坏死概率,为移位型跟骨关节内骨折推荐治疗方法。Objective To compare the therapeutic efficacy of percutaneous reduction and fixation and open reduction and fixation for displaced intra-articular calcaneus fractures.Methods From march 2008 to January 2010,40 patients with displaced intra-articular calcaneus fractures were recruited in this study.Under randomization,20 were assigned into the percutaneous reduction and fixation group,20 into the open reduction and fixation group.Results There were no significant differences in the Bohler’s angle,the calcaneus width,the calcaneus length between the two groups.The incidence of wound complications was none of 20 in the percutaneous reduction and fixation group and six of 20 in the open reduction and fixation group（P〈0.05）.Conclusion The results of this study suggest that in comparison to open reduction and fixation,percutaneous reduction and fixation may be a better choice.
