肾上腺髓质素对大鼠肾脏损伤后AngⅡ及TNF-α表达的影响

摘  要:目的观察外源性肾上腺髓质素(ADM)对肾脏机械性损伤后血管紧张素Ⅱ(AngⅡ)、肿瘤坏死因子-α(TNF-α)表达的影响,探讨其对肾脏损伤的保护作用。方法健康成年普通级Wistar大鼠104只,随机分为4组:正常对照组(8只)、单纯创伤组(32只)、伤前给药组(32只)、伤后给药组(32只)。后3组采用自由落体打击仪直接打击大鼠脊肋区制作肾脏机械性损伤模型;两给药组分别于创伤前后10 min腹腔注射ADM(0.1 nmol/kg);3个肾脏损伤组平均分为4批分别于创伤后1、6、12、24 h采用快速心脏采血法处死。迅速解剖动物提取肾脏标本,放免法检测肾组织匀浆上清液中AngⅡ及TNF-α水平,并进行统计学分析。结果①AngⅡ水平:伤前给药组在1 h6、h点明显降低,在12 h点明显升高,与该时点单纯创伤组相比差异有统计学意义(P<0.05,P<0.01)。伤后给药组在伤后1 h降低,与单纯创伤组相比差异有统计学意义(P<0.05);其他各时点无明显差异。②TNF-α水平:伤前给药组在1 h、6 h时点无明显改变,12 h及24 h时点表达明显降低,与该时点单纯创伤组相比差异有统计学意义(P<0.01);伤后给药组同伤前给药组。结论 AngⅡ及TNF-α在肾脏损伤与修复中主要发挥损害作用,外源性ADM可下调二者的水平,分别在肾脏急性损伤的早期和后期发挥保护作用。Objective To observe the effects of exogenous adrenomedullin(ADM) on the expressions of angiotensin Ⅱ(AngⅡ) and tumor necrosis factor-alpha(TNF-α) after mechanical renal trauma so as to explore ADM's protective effect on renal trauma.Methods A total of 104 healthy adult Wistar rats of plain grade were randomly divided into four groups: control group(n=8),trauma group(n=32),pre-trauma ADM injection group(n=32) and post-trauma ADM injection group(n=32).Eight rats in the last three groups were killed at 1 h,6 h,12 h and 24 h after trauma,respectively.The model of mechanical renal trauma was prepared by striking the area of rat skin reflected by the kidney with free dropping ferreous hammer.ADM(0.1 nmol/kg) was injected intraperitoneally at 10 minutes before and after trauma,respectively.All rats were executed by withdrawing all the blood in their hearts under deep anesthesia.The concentrations of TNF-α and Ang II were detected in the supernatant of renal tissue homogenates by radioimmunoassay.Results AngⅡ concentration decreased significantly at 1 h and 6 h,but increased significantly at 12 h in pre-trauma ADM injection group as compared with that in trauma group(P〈0.05,P〈0.01).In post-trauma ADM injection group AngⅡ decreased obviously at 1 h after trauma compared with that in trauma group(P〈0.05),but did not differ significantly at the other time points.TNF-α concentration did not change significantly at 1 h and 6 h in pre-trauma ADM injection group as compared with that in trauma group,but decreased obviously at 12 h and 24 h(P〈0.01).It was the same case with post-trauma ADM injection group.Conclusion AngⅡ and TNF-α play an antagonistic role in renal damage and repair.Exogenous ADM can down-regulate the expressions of AngⅡ and TNF-α,thus having protective effects on early-and late-stage renal acute injury.
