摘　　要：目的探讨实时三维超声心动图(RT-3DE)定量评价慢性心力衰竭(CHF)患者左心室收缩同步性的临床价值。方法将研究对象分为心衰组(41例)和正常对照组(30名),并行TDI和RT-3DE检查,应用QLab软件分析TDI及三维全容积数据,对两种检查方法所获结果进行统计学分析。结果两组患者年龄和心率差异均无统计学意义(P>0.05),左心室射血分数及QRS波差异有统计学意义(P<0.05)。两种评价方法的收缩非同步性指数(SDI)差异有统计学意义(P<0.05)。两种评价方法的SDI呈中度正相关,具有较高的一致性。结论 RT-3DE可定量分析左心室收缩同步性,为心脏再同步化治疗术前筛选心力衰竭患者提供一种简单、快捷的新方法。Objective To explore the clinical value of real-time three-dimensional echocardiography（RT-3DE） on qualitative evaluation of intra-left ventricular synchronism in patients with chronic heart failure（CHF）.Methods Forty-one patients with CHF were included in CHF group,while 30 normal subjects were enrolled in control group（n=30）.TDI and RT-3DE data were acquired with an iE33 system and analyzed by the QLab software.Results No difference of age or heart rate was found（P0.05）,while left ventricular ejection fraction and QRS waves were significantly different between the two groups（P0.05）.Systolic dyssynchrony index（SDI） of the two evaluation methods were significantly different（P0.05）.Moderate positive correlation and higher concordance were documented between the two methods in evaluating SDI.Conclusion RT-3DE can quantify global left ventricular synchronism,may be used as a simple and quick method to identify chronic heart failure patients before CRT.
