摘  要:目的:研究阴道超声监测宫颈情况联合血β-hCG值在预测早产方面的可行性及价值。方法:选择2009年5月~2010年12月收治的268例先兆早产孕妇为A组,孕周28~32周,单胎头位,出现阴道流血和(或)腰酸和(或)腹痛,宫口开大<3cm;同时选择268例先兆早产孕妇为B组,孕周33~36周,单胎头位,出现阴道流血和(或)腰酸和(或)腹痛,宫口开大<3cm;两组均排除前置胎盘、胎盘早剥及曾接受过宫颈手术的患者。待两组先兆早产孕妇排空膀胱后,取膀胱截石位,常规消毒外阴行阴道超声检查,同时检测静脉血β-hCG水平。结果:阴道超声监测宫颈长度联合血β-hCG值检测在预测早产的灵敏度、特异度、阴性预测值、阳性预测值方面均较高。结论:阴道超声监测宫颈长度联合血β-hCG值能较准确地预测早产。Objective:To study the vaginal ultrasound monitoring cervical situation joint blood beta hCG value in the forecast the feasibility and value of the premature birth.Methods:From May 2009 to December 2010 268 patients were admitted to a threatened preterm group,28 to 32 weeks gestational age,single cephalic,vaginal bleeding and(or) backache,and(or) abdominal pain,cervix large 〈3 cm;The same time 268 cases of threatened preterm selected for the B group,33 to 36 weeks gestation,single cephalic,vaginal bleeding and(or) backache,and(or) abdominal pain,cervix large 〈3 cm;Two groups were excluded before home placenta,placental abruption and had received cervical surgery patients.Be two groups of pregnant women threatened premature labor after emptying the bladder,bladder lithotomy position to take,conventional disinfection genital transvaginal ultrasonography,simultaneous detection of blood β-hCG level.Results:The vaginal ultrasound monitoring cervical length joint blood beta hCG value in the prediction of premature detection sensitivity,specific degrees,negative prediction were higher,and the masculine predicted.Conclusion:Transvaginal ultrasound cervical length monitoring of blood β-hCG value of the joint can more accurately predict preterm birth.
