带锁髓内钉治疗胫骨骨折疗效观察

摘　　要：目的:观察带锁髓内钉与AO钢板在治疗胫骨骨折中的疗效。方法:回顾性分析本院2007年10月～2010年10月收治的70例胫骨骨折患者的临床资料。根据不同的治疗方法,将患者分为两组,A组32例采用带锁髓内钉固定治疗,B组38例采用AO钢板内固定,观察两组患者的疗效和并发症,并比较两组患者的骨痂出现时间,下肢负重时间和骨折愈合时间。结果:两种不同的方法治疗胫骨骨折,疗效和并发症比较差异有统计学意义。骨痂出现时间差异无统计学意义(P>0.05),下肢负重时间和骨折愈合时间差异有统计学意义(P<0.05)。结论:带锁髓内钉防旋性能好,内固定牢固;主钉位于长骨中线,符合生物力学;手术创伤小,有利于促进骨折愈合;早期进行功能锻炼,避免了关节功能障碍的发生。Objective：To observe the effect of intramedullary nailing and AO steel to treat tibial fracture.Methods：70 cases with tibial fracture from October 2007 to October 2010 in our hospital were analyzed retrospectively.According to the different treatment,patients will be divided into two groups,32 cases of A group adopted fixation of belt intramedullary nails,38 cases of B group adopted fixation of AO plate,to observe the curative effect and complications of two groups,and compare the appear time,lower limb load time and fracture healing time of two groups.Results：The comparative differences of efficacy and complications of two different treatments had statistical significance.The appear time of callus had no statistical significance（P0.05）,lower limb load time and fracture healing time of two groups had statistical significance（P0.05）.Conclusion：The treatment of intramedullary nails prevent the spin performance well,internal fixation is secure,the main nail is locate in centerline of bone according with biomechanical;the surgical has small trauma,is helpful for promoting fracture healing,early functional exercise can avoid the occurrence of the joint dysfunction.
