摘  要:目的评价超声引导下经皮肾镜碎石术(PCNL)治疗上尿路结石的疗效和治疗经验。方法回顾性分析2007年12月至2009年11月采用PCNL治疗的207例上尿路结石患者的临床资料。其中单纯肾结石141例,肾结石合并输尿管结石25例,单纯输尿管结石28例,多发结石180例,铸型结石68例。结石大小2.64~30.21 cm3。所有患者均用EMSⅢ代气压弹道联合超声组合式碎石系统碎石。结果所有患者1次穿刺成功,188例患者1次取石成功,19例患者行二期PCNL手术成功。手术时间为50~130 min,平均90 min,术后平均住院时间6 d。结石清除率为92%。术后并发症71例,其中出血34例,发热37例。结论超声引导下标准通道PCNL治疗上尿路结石创伤小,术后恢复快,安全有效。Objective To present the clinic efficacy and our experience with PCNL using pneumatic and ultrasonic power under B-type ultrasound guidance in the treatment of calculus in upper urinary.Methods We summarized the clinical data of 207 cases treated by standard PCNL from December 2007 to November 2009.The indications for PCNL were renal stones in 141 cases,renal stones with ureteral stones in 25 cases,ureteral stones in 28 cases,multiple calculus in 180 cases,emplastic calculus in 68 cases.The maximum of stone burden was 30.21 cm3.Operations were performed by EMS Ⅲ LithoClast Master.Results The punctures were completed successfully in all cases,188 of 207 cases were performed in one-session,19 cases were performed successfully in two-session.The mean operation time was 90(50 to 130)minutes,the mean hospital stay was 6 days.The stone-free rate was 92%.Major complications were seen in 71 PCNL procedures which was included in significant hematuria requiring blood transfusion in 34 cases,fever in 37 cases.Conclusion Standard PCNL with pneumatic and ultrasonic power under B-type ultrasound guidance was a safe and effective procedure in treatment of calculus in upper urinary.
