摘  要:目的探讨骨髓间充质干细胞(Bone marrow mesenchymal stem cells,BMSCs)移植和直流电刺激对大鼠脊髓损伤(Spinal cord injury,SCI)的修复作用。方法采用脊椎骨破坏法制备大鼠SCI模型,将模型大鼠随机分为模型组、BMSCs移植组、电刺激组和电刺激+BMSCs移植组,通过BBB评分、体感诱发电位(Somatosensory evoked potential,SEP)测定和免疫组化法观察不同方法治疗SCI的效果。结果 BMSCs移植后7和14 d,模型组和电刺激组BBB评分与BMSCs移植组和电刺激+BM-SCs移植组相比,差异有统计学意义(P<0.05);电刺激+BMSCs移植组脊髓功能恢复最好。SEP检测,BMSCs移植后14 d,BMSCs移植组和电刺激+BMSCs移植组与其他两组比较,潜伏期幅有明显差异(P<0.05)。采用NF-200标记脊髓神经元,模型组在损伤14 d后,仅见少量的NF阳性细胞;而电刺激组和BMSCs移植组可见大量NF阳性细胞和轴突分布于损伤区周围;电刺激+BMSCs移植组NF阳性细胞明显多于电刺激组和BMSCs移植组(P<0.05)。BMSCs移植组在移植后7 d,局部组织脑衍生的亲神经因子(Brain-derived neurotropic factor,BDNF)表达量明显高于模型组(P<0.05),14 d最强,21 d减弱,电刺激+BMSCs移植组BDNF表达最强(P<0.05)。结论 BMSCs移植联合电刺激能够最大程度地促进SCI修复。Objective To investigate the repair effect of transplantation with bone marrown mesenchymal stem cells(BMSCs) and stimulus of direct current(DC) on spinal cord injury(SCI) in rats.Methods Rat model of SCI was established by damage of spine.The model rats were randomly divided into model,BMSC transplantation,DC stimulation and DC stimulation + BMSC transplantation groups,and observed for curative effect by BBB scoring,determination of somatosensory evoked potential(SEP) and immunohistochemical assay.Results Seven and fourteen days after transplantation with BMSCs,the BBB scores in model and DC stimulation groups showed significant difference with those in BMSC transplantation and DC stimulation + BMSC transplantation groups(P 0.05).Compared with those in other groups,the spinal function of rats in DC stimulation + BMSC transplantation group was recovered satisfactorily.The determination result of SEP proved that the evoled potential latencies of BMSC transplantation and DC stimulation + BMSC transplantation groups 2 weeks after transplantation showed significant difference with those in the other two groups(P 0.05).Immunohistochemical assay showed only a small quantity of NF-positive cells in model group 14 d after injury,while a large quantity of NF-positive cells and axons were found around the injured regions in DC stimulation and BMSC transplantation groups.The NF-positive cell count in DC stimulation + BMSC transplantation group was significantly larger than those in DC stimulation and BMSC transplantation groups(P 0.05).The expression level of brain-derived neurotropic factor(BDNF) in local tissues of rats in BMSC transplantation group was significantly higher than that in model group 7 d(P 0.05),and reached a peak value 14 d while decreased 21 d after transplantation.However,the expression level of BDNF in DC stimulation + BMSC transplantation group was higher than those in other groups(P 0.05).Conclusion Transplantation with BMSCs and stimulus of DC maxim
