子宫腔输卵管DSA造影研究

摘　　要：目的利用不同的注射方法、推注不同浓度的泛影葡胺,行子宫腔输卵管DSA造影,对图像质量、X线辐射剂量、造影检查不良反应等方面进行对比研究。方法 153例子宫腔输卵管DSA造影患者:研究组53例,利用高压注射器、推注50%泛影葡胺造影。对照组1患者50例,手推76%泛影葡胺造影。对照组2患者50例,利用高压注射器、推注76%泛影葡胺造影。统计图像显示情况、患者体表剂量面积乘积、造影检查不良反应等方面数据,推算出单个卵巢裸露在骶髂部体表时的剂量面积乘积、吸收剂量、有效剂量的平均值三个数据,并进行统计学分析。结果研究组与对照组在显示子宫腔及输卵管各段等图像质量方面差异无统计学意义。在平均子宫腔显影时间、平均采集DSA图像数量、患者卵巢辐射剂量方面,研究组优于对照组,差异有统计学意义。在造影检查不良反应方面,研究组中的患者好于对照组。结论利用高压注射器推注低浓度对比剂,行子宫腔输卵管DSA造影检查,图象质量没有降低,检查操作稳定性提高,造影检查不良反应减轻,患者X线辐射剂量减少,可以避免医生辐射损伤。Objective To compare the image quality,X-ray radiation dose,adverse effect in hysterosalpingography of DSA using different injection methods and different concentrations of diatrizoate.Methods One hundred and fifty-three cases who received hysterosalpingography of DSA were divided into three groups.Study group（53 patients） were injected 50% diatrizoate contrast using high-pressure syringe.Control group 1（50 patients） were pushed 76% diatrizoate contrast and control group 2（50 patients） were injected 76% diatrizoate contrast using high-pressure syringe.The informations of images,the patient body surface dose area product and hysterosalpingography adverse reactions were counted and analyzed.The average of dose area product,the average of absorbed dose,the average of effective dose when a single ovary exposed surface were calculated and analysed according to patient surface dose area product.Results There was no marked difference between the study group and control groups on image quality.The study group was much better than control groups on the average imaging time of uterine cavity,the average number of DSA images,radiation dose of ovary and hysterosalpingography adverse reactions.Conclusion The using of high-pressure syringe with low concentrations of contrast agent bolus in the uterine cavity and oviduct DSA angiography would improve the X-Ray radiation damage on patients and doctors,which will not reduce the image quality,and will heighten operational stability,and will reduce adverse reactions.
