宫腔镜直视下输卵管注射MTX配合米非司酮治疗输卵管妊娠

摘　　要：[目的]观察宫腔镜直视下输卵管注射甲氨蝶呤(MTX)配伍米非司酮治疗输卵管妊娠的疗效。[方法]选取收入我院治疗的80例输卵管妊娠患者,随机分为2组,试验组是指在宫腔镜直视下输卵管注射甲氨蝶呤配合米菲斯酮治疗输卵管妊娠,该组随机分40人;对照组是指臀部肌肉注射甲氨蝶呤并配合米非司酮治疗输卵管妊娠,该组也随机选取40人。所有患者均由同一专业人员定期检查β-hcg和输卵管处包快大小。比较2组的治疗效果。[结果]试验组中有38例治疗成功,2例由保守治疗转到手术治疗,治愈率为95%(38/40),血β-hcg下降例数有38例,占95%,血β-hcg转阴时间为(11.5±5.3)d,有35例包快缩小程度大于30%;而对照组治愈率为80%(32/40);血β-hcg下降例数有32例,占80%,血β-hcg转阴时间为(14.1±4.2)d,有27例患者包快缩小大于30%。试验组和对照组的数据资料,经卡方检验和t检验分析,P值均小于0.05,差异有统计学意义。[结论]在宫腔镜直视下输卵管注射甲氨蝶呤配伍米非司酮治疗输卵管妊娠比经臀部肌肉注射MTX配伍米非司酮治疗输卵管妊娠效果好。患者治愈率高,恢复快,生活质量高。[Objective] To observe the treatment effect of salpingocyesis with injecting methotrexate （MTX） in tubal by the hysteroscope directly with mifepristone. [Methods] A total of 80 cases of our treatment salpingocyesis patients were selected, and were randomly divided into two groups. Patients were injected with MTX in tubal by the hysteroseope directly with mifepristone, and this group had 40 people; Control group got coxal muscle injection methotrexate with mifepristone and had 40 people. All of the patients were detected by the same professional regular inspection with beta HCG and tubal bag fast in size. We compared the outcome of the two groups treatment. [Results] 38 cases of patients were successful treatment, 2 cases were turned to surgery, and the cure rate was 95% （38/40）, a number of 38 casesofβ-HCG were down, accounted for 95%, β-HCG turn Yin time on 11.5 ± 5.3 days, and the lump of 35 cases were faster to a greater extent than 30% ; reduce and control group cure rate was 80% （32/40） ; a number of 32 cases ofβ-HCG were down, accounted for 80%, β-HCG turn Yin time was on 14.1 ± 4.2 days, and the lump of 27 patients was fast to a greater extent than 30%. Patients in the control group and the data material, the chi-square test analysis, and t p value was less than 0.05, the differences were more significant. [Conclusion] The treatment effect of salpingocyesis with injecting methotrexate （MTX） in tubal by the hysteroscope directly with mifepristone was better than that of the coxal muscle. Patients had high cure rate, faster recovery, high quality of life.
