摘 要:[目的]对2005~2009年丰台区细菌性痢疾的病原菌型和最新的耐药性进行监测和分析,以更好的指导临床的诊断和用药。[方法]连续5年对临床诊断为细菌性痢疾的病人进行粪便培养,并对培养阳性的菌株进行分型鉴定,并对2009年获得的阳性菌株进行19种最新抗生素的药敏鉴定。[结果]从2005~2009年丰台区宋内和福氏菌株分布来看,优势菌型出现了明显变迁,宋内逐渐取代了福氏(χ2=45.58,P﹤0.01),而2009年的菌株耐药性鉴定结果显示多重耐药性严重,均在4重耐药以上,其中4重耐药和5重耐药的菌株占到94%。[结论]随着卫生条件的改善,引起细菌性痢疾的优势菌株也发生了变化,新菌株的药敏谱也发生了变化,医疗机构和疾病预防控制部门应根据这种变化,将医疗和预防措施进行适当的调整。[Objective] To monitore the shigella flexneri about its type and antimicrobial resistance from 2005 to 2009 in Fengtai District, and offer better instruction for clinical diagnosis and medication. [Methods] In last five years, we monitored those patients diagnosed as bacillary dysentery based on whose clinical signs, and collected their excrement samples for bacterial culture and distinguish the bacterial type. Furthermore, we tested the drug resistance of the latest shigella flexneri collected in :2009. [Results] According to these experimental results, we can learn that the dominating bacterial type has changed obviously in last five years. There was a tendency for Sh. flexneri to be, replaced by Sh. Sanni (χ2= 45.58, P 〈 0.01 ). At the same time, the antimicrobial resistance of these shigella flexneri collected in 2009 had become very severe, 100% of these shigella flexneri had the ability to resist more than four drugs, and 94% of them could resist to four or five drugs. [Conclusion] With the improvement of health conditions, dominating type of shigeUa flexneri has changed, the same was their antimicrobial resistance, medical institutions and Centers for Disease Control and Prevention should adjust their working measures appropriately to adapt to the changing.
