吡喹酮透皮给药治疗日本血吸虫病

摘　　要：目的探究吡喹酮（praziquantel，PZQ）透皮剂的治疗效果，以及吡喹酮与宿主免疫系统是否具有协同作用，为治疗血吸虫病提供更便利的治疗途径。方法新西兰大白兔感染日本血吸虫尾蚴150±2条或300±2条，透皮剂浓度为24％，以100mg／kg剂量对感染El本血吸虫28d的新西兰大白兔分别进行腹部透皮治疗1次和2次，并在1次感染和2次感染后1d和14d透皮治疗2次。统计治疗组的减虫率、肝减卵率、减毛蚴孵化率，评价透皮剂的治疗效果。结果感染日本血吸虫28d后吡喹酮透皮剂透皮治疗2次效果优于1次，减虫率分别为96．23％和80．19％，肝减卵率和减毛蚴孵化率均达到或接近100％。对感染后1d和14d的透皮治疗，感染2次并治疗的减虫率分别为86．62％和91．84％。结论24％的PZQ透皮制剂具有较好的杀灭日本血吸虫成虫作用，透皮次数越多，治疗效果越好；透皮剂对杀灭日本血吸虫童虫也有一定的作用，对感染2次并治疗的减虫效果更好，可能与感染宿主的免疫协同作用有关。Objective To explore the treatment effect of praziquantel.（PZQ） transdermal agent, and whether praziquantel and host immune system having synergistic effect in treatment of schistosomiasis. Methods The New Zealand white rabbits were infected with Schistosoma japonicum 150 ± 2 or 300± 2 cereariae. The concentration of transdermal agent was 24%. The rabbits were treated through abdominal transdermal once and twice at the dose of 100 mg/kg on day 28 post infection, or they were treated twice on day 1 or day 14 post infection in secondary infection. Worm reduction rate, egg reduction rate and miracidia hatching reduction rate of treatment groups were calculated to evaluate the therapeutic effect of transdermal agent. Results The effect of praziquantel transdermal therapy twice was better than once on day 28 post infection. Worm reduction rates were 96.23% and 80. 19% respectively. Egg reduction rate and miraeidia reduction rate were at or close to 100%. The worm reduction rates of treatment on day 1 or day 14 after secondary infection were 86.62% and 91.84% respectively. Conclusion 24% of praziquantel transdermal agent had good effect on adult Schistosorna japonicum. The more transdermal therapy times, the better treatment effect. Transdermal agent also had effect on sehistosomulum, and had the better worm reduction rate on secondary infection after treatment than first infection, indicating that praziquantel may have immune-synergy effect.
