失眠患者自主神经功能障碍的临床研究

摘　　要：目的探讨失眠患者自主神经功能障碍的发生率,以及自主神经功能障碍与患者的发病年龄、性别、病程、病情严重程度、交感神经皮肤反应等因素的相关性,尤其是皮肤交感反应是否可成为判断失眠患者严重程度及自主神经功能障碍的敏感指标。方法对48例病例组失眠患者进行汉密尔顿焦虑量表(HAMA)、抑郁量表(HAMA/HAMD)、自主神经症状量表(ASP)、匹茨堡睡眠质量指数量表(PSQI)评分和皮肤交感反应(SSR)评定,另选47例性别、年龄、文化程度相匹配的健康者作为对照组。结果①病例组与对照组ASP、SSR异常率比较,P值分别为0.015和0.02,2组比较均有显著性差异;②病例组ASP与SSR,ASP与PSQI比较,P值分别为0.001和0.000,相互间存在统计学相关性。结论①ASP可用于失眠患者自主神经功能症状评估;②SSR可作为反映失眠患者自主神经功能早期损害的较灵敏指标。Objective It is to approach the incidence of autonomic dysfunction of insomnia patients,and autonomic dysfunction in patients with age of onset,gender,disease duration,disease severity,degree of sympathetic skin response and other factors related;particularly if a sympathetic skin response diagnosis of the severity of patients with insomnia and autonomic dysfunction sensitive indicators.Methods forty-eight cases of insomnia patients group Hamilton Anxiety and Depression Scale（Hama/Hamd）,Pittsburgh sleep quality index（PSQI）,autonomic symptom profile（ASP） and sympathetic skin response（SSR） assessment,choose another forty-seven cases of sex,age,education level matched healthy subjects served as controls.Results ① Compared the abnormal rate of ASP and SSR in Case group with control group,P values were 0.015 and 0.02 respectively,and the difference was significant;②In case group,compared the value of ASP with SSR,and ASP with PSQI,P values were 0.001 and 0.000 respectively,There were statistical correlations between them.Conclusion ①ASP can be used to assesse autonomic nerve function for patients with insomnia symptoms;②SSR may be a sensitive indicator for early damage of autonomic nerve function in patients with insomnia.
