摘 要:目的观察氦氧混合气为驱动气源雾化吸入支气管舒张剂和激素对慢性阻塞性肺疾病患者疗效。方法选择19例慢性阻塞性肺疾病急性发作的住院患者,随机分为实验组(11例,以氦氧混合气为驱动气源)和对照组(8例,以氮氧混合气为驱动气源)雾化吸入硫酸沙丁胺醇和布地奈德混悬液,观察治疗前后的变化。结果两组在吸入气味、流速,下呼吸道感觉,雾化时咳嗽,咳痰难度,鼻咽部感觉,生命体征,动脉血二氧化碳分压,痰液性状,痰量,干湿啰音方面,差异无统计学意义(P>0.05),两组在雾化时间、吸入难度、憋气、动脉氧分压、呼吸困难方面,差异有统计学意义(P<0.05),实验组雾化时间明显缩短,氧分压提高显著,呼吸困难症状明显改善。结论氦氧混合气为驱动气源雾化吸入支气管舒张剂和吸入激素在慢性阻塞性肺疾病急性发作治疗方面与传统方法比较有明显优势,缩短雾化时间,提高吸入药物的利用率,改善缺氧及呼吸困难症状。Objective To observe the therapeutic effect of helium and oxygen aerosol inhalation on chronic obstructive pulmonary disease(COPD).Methods Nineteen hospitalized patients with COPD were randomly divided into experimental group(n=11) and control group(n=8).Patients in experimental group inhaled helium and oxygen aerosol and those in control group inhaled nitrogen and oxygen aerosol to observe changes of their symptoms before and after treatment.Results No significant difference was observed in smell and flow speed of inhaled aerosol,feeling of lower respiratory tract,cough,expectoration,feeling of nasopharynx,vital signs,PaO2,property and volume of sputum,dry and wet rales between the two groups.However,significant differences were found in aerosolized time,inhalation difficulty,respiration holding,PaO2,and dyspnea between the two groups(P0.05).The aerosolized time was shorter,the PaO2 was significantly higher,and the improvement of dyspnea symptoms was better in experimental group than in control group(P0.05).Conclusion Inhalation of helium and oxygen aerosol is significantly advantageous over the traditional method for COPD by shortening the aerosolized time,increasing the availability of inhaled drugs,and improving the symptoms of hypoxia and dyspnea.
