经皮穴位电刺激合谷足三里对单肺通气所致炎性反应的抑制作用

摘　　要：目的:观察经皮穴位电刺激合谷足三里对开胸术单肺通气所致炎性反应的抑制作用。方法:择期开胸者42例随机分为经皮穴位电刺激组(E组)和对照组(C组)。E组在麻醉诱导前30 min开始经皮电刺激两侧合谷、足三里。分别于插管后30 min(T0)、单肺通气结束(T1)、术毕(T2)取支气管灌洗液检测炎性介质肿瘤坏死因子α(TNF-α)、白介素1β(IL-1β)、白介素8(IL-8)、白介素10(IL-10)及白细胞分类计数。结果:IL-1β及IL-8:E组在T2时点高于T0(P<0.05),C组T1、T2均高于T0(P<0.05),T1、T2时点组间比较,E组低于C组(P<0.05);白细胞计数及粒细胞百分比:与T0时点比较,C组在T1、T2明显升高(P<0.05),而E组仅在T2略有升高(P<0.05);组间比较,C组在T2时点明显高于E组(P<0.05)。结论:经皮电刺激合谷足三里穴位可以通过抑制单肺通气时炎性介质IL-1β、IL-8及白细胞、粒细胞的反应而减少肺损伤。Objective：To observe the effects of transcutaneous electrical acupoint（TEAS）stimulation of Hegu and Zusanli upon pulmonary inflammatory responses induced by one lung ventilation.Methods：42 patients scheduled for thoracotomy were randomized into TEAS group（E group）and control group（C group）,with 21 cases in each.Patients in E group were administrated with TEAS of Hegu and Zusanli 30 min before anesthesia induction.Bronchoalveolar lavage（BLA） was performed 30 min before induction（T0）,the end of one lung ventilation（T1）,operation completion（T2）.The concentration of TNF-α,IL-1β,IL-8,IL-10 and count of cells in BAL fluid were determined.Results：IL-1β and IL-8：E group was higher at T2 than T0（P0.05）,C group was higher at T1,T2 than T0（P0.05）,E group was lower at T1,T2,compared with C group（P0.05）;white blood cell count and granulocyte percentage：compared with T0,C group increased at T1,T2（P0.05）,E group increased only at T2（P0.05）;compared within groups,C group was higher than E group at T2 significantly（P0.05）.Conclusions：TEAS of ＂Hegu＂ and ＂Zusanli＂ may reduce pulmonary injury by suppress the responses of inflammatory mediator IL-1β,IL-8 and WBC,granulocytes during one lung ventilation.
