摘  要:目的观察影响新生儿鼻塞持续气道正压通气(n-CPAP)过程中导致压力不稳定的因素。方法对2006年10月—2009年8月新生儿科148例使用n-CPAP辅助呼吸的新生儿,记录导致n-CPAP压力不稳定的因素,包括鼻塞滑脱、鼻塞堵塞、管路冷凝水过多、患儿躁动不安、固定不妥、鼻塞过小、管路漏气等,并进行Logistics回归分析。结果鼻塞滑脱、鼻塞堵塞、管路冷凝水过多、患儿躁动不安、固定不妥与n-CPAP压力异常显著相关。结论鼻塞脱落、鼻塞堵塞、管路冷凝水过多、患儿躁动不安、固定不妥为引起压力异常的主要因素,应予以及时、及早排除。Objective To observe the factors associated with pressure instability of nasal continuous positive airway pressure(CPAP)received by neonates.Methods From Oct.2006 to Aug.2009,148 neonates receiving nasal CPAP were observed.The factors possibly associated with pressure instability of nasal CPAP were collected,including prong displacement,prong obstruction,water condensation inside CPAP hose,restlessness,unfitted headgear,improper prong size and excessive leakage.The Logistic regression analysis on the factors resulting in pressure instability were performed.Results The pressure instability during nasal CPAP was related to prong displacement,prong obstruction,water condensation inside CPAP hose,restlessness and unfitted headgear.Conclusions The factors resulting in pressure instability during nasal CPAP include prong displacement,prong obstruction,water condensation inside CPAP hose,restlessness and unfitted headgear.All these factors should be avoided during nasal CPAP.
