摘　　要：目的探讨不同临床病期的慢性HBV感染者外周血单个核细胞(PBMC)中T淋巴细胞亚群及NK细胞活性变化。方法收集慢性乙型肝炎、慢性重型乙型肝炎、乙型肝炎肝硬化、肝癌四组患者及健康人群的外周血,应用流式细胞术检测CD8+T细胞、CD4+T细胞及NK细胞的表达量和分泌细胞因子的能力,并检测NK细胞的杀伤活性。结果各组患者CD8+T细胞计数与健康对照组比较均有所下降;但CD4+T细胞计数较健康对照组均无显著变化;NK细胞计数除慢性乙型肝炎组外均显著低于健康对照组(P<0.01)。各组患者NK细胞分泌IFN-γ、TNFα-的能力及CD8+T细胞分泌TNFα-的能力均低于健康对照(P<0.05)。NK细胞杀伤活性随疾病进展程度有逐渐减低的趋势(健康对照组>慢性乙型肝炎组>慢性重型肝炎组>肝硬化组>肝癌组)。结论慢性HBV感染者NK细胞功能降低,且与病情进展有一定关联。Objective To investigate the change of T-lymphocyte subsets and NK cell activity in PBMC in patients with HBV infection of different clinical stage.Methods Blood samples were collected from the patients with chronic hepatitis B,chronic severe hepatitis,liver cirrhosis,and liver cancer as well as healthy persons.Flow cytometry assay was used to detect the expression of CD8＋ T cells,CD4＋ T cells,NK cell and the activities of cytokine production and NK cell cytotoxicity.Results Compared with controls,the number of CD8＋ T cells decreased in the patients,but the number of CD4＋ T cells showed no significant difference.Compared with controls,the number of NK cells of the patients decreased markedly（P0.01）except chronic hepatitis B.Lower frequencies of NK cells from the patients secreted interferon-γ and NK cells,CD8＋ T cells from the patients secreted tumor necrosis factor-α compared with normal controls（P0.05）.NK cell cytotoxicity had a downward trend following the degree of disease progression（healthy personschronic hepatitis Bchronic severe hepatitisliver cirrhosisliver cancer）.Conclusion The activity of NK cells in patients with chronic HBV infection decreased the deterioration of liver disease.
