摘　　要：目的应用ROC曲线评价子宫动脉多普勒参数对异常妊娠结局的预测价值。方法定群分析妊娠期高血压疾病83例,并随访至产后,以妊娠异常结局:早产,小于孕龄儿(SGA),剖宫产为"金标准",绘制子宫动脉参数:子宫动脉搏动指数(PI)、阻力指数(RI)、收缩期与舒张期流速比(S/D)和子宫动脉评分对异常结局的ROC曲线并计算曲线下面积(AUC)和最佳诊断界值。结果子宫动脉参数PI是预测异常妊娠结局的最佳参数,稍高于RI值,子宫动脉评分对SGA的预测能力最高。结论妊娠期高血压疾病中子宫动脉参数PI、RI及子宫动脉评分对异常妊娠结局有较好的预测价值。Objective To assess the predictive value of Abnormal pregnancy outcome with Doppler parameter of uterine artery by receiver operating characteristic Curves （ROC）. Methods A cohort study including 83 patients of hypertensive disorder complicating pregnancy, were followed up to postnatal, and ROC of uterine artery blood flow parameters drawed for the adverse Pregnancy Outcome of small for gestational-age （SGA）, newborns Premature birth, Cesarean section. The parameters including pulsatility indices（H）, resistance indices（RI）, systolic to diastolic velocity ratios（S/D） and uterine artery scores were calculated the area under ROC curve （AUC） and the cut off values. Results The pulsatility index （PI） of uterine artery was the best predictors of adverse outcome of pregnancy, slightly higher than those with RI, and uterine artery scores has the highest capability of predicting SGA. Conclusions PI,RI and uterine artery score were of better predictive value for abnormal pregnancy outcome in patients with hypertensive disorder complicating pregnancy.
