急性前交叉韧带实质部完全断裂的发生部位及有关因素研究

摘　　要：目的探讨急性前交叉韧带（anterior cruciate ligament,ACL）实质部完全断裂的发生部位,及其与5种主要损伤有关因素的关系。方法 1988年5月-2005年12月,144例急性ACL损伤（受伤时间1-21 d）经关节镜探查确认韧带实质部完全断裂,按照长度将实质部三等分分为上、中、下三部分,观察断裂部位与年龄、性别、损伤侧别、损伤机制（接触性、非接触性）和损伤时膝关节位置（伸直、屈曲）的关系。结果上、中、下三段断裂的发生率分别为：72.2%（104/144）、26.4%（38/144）、1.4%（2/144）,两两比较均有统计学差异（P〈0.05）。上段和中段断裂在不同年龄、性别、损伤侧别、损伤机制和损伤时膝关节位置的发生率比较均无统计学差异（P〉0.05）。结论急性ACL实质部完全断裂近3/4发生在上段,1/4左右发生在中段,下段极少发生;断裂部位与年龄、性别、损伤侧别、损伤机制以及损伤时膝关节位置均无关。Objective To investigate the position of acute complete substance tears of the anterior cruciate ligament（ACL） and five relevant factors.Methods A total of 144 cases with acute ACL injuries（1-21 days after the injuries） were confirmed having sustained complete substance tears by arthroscopy and received arthroscopic reconstruction from May 1988 to December 2005.Each ligamentous substance was divided into three portions with equal length： proximal,middle and distal portion.The torn position of each case was recorded.The age,gender,affected side,mechanism of the injury and knee joint direction when injury occurred,as well as relationship between torn positions and each of the five relevant factors were tested using the Chi-square test.Results Incidences of occurrence of proximal,middle and distal injury were 72.2%（104/144）,26.4%（38/144）,and 1.4%（2/144）,respectively（P0.05）.No significante difference existed in age,sex,affected side,injury mechanism,and the knee position at jnjury between the injuries to the upper and middle portions（P0.05） Conclusions Acute complete substance tears of the ACL mostly happen in the proximal portion（approximately 3/4）.The torn position has no statistical significant relationship with any of the five relevant factors.
