经尿道前列腺等离子双极电切术与常规电切术治疗良性前列腺增生症疗效比较

摘　　要：目的:比较经尿道前列腺等离子体双极电切术(TUPKRP)和经尿道前列腺电切术(TURP)治疗良性前列腺增生症的安全性和可行性。方法:82例良性前列腺增生症患者随机分为两组,分别行TUP-KRP(45例)和TURP(37例),比较两组手术时间、术中出血量、住院时间、冲洗时间、留管时间以及国际前列腺症状评分(IPSS)、生活质量评分(QOL)、术前最大尿流率(Qmax)和剩余尿量(PVR)以及并发症发生情况。结果:TUPKRP组失血量明显少于TURP组(P<0.05),且术后恢复时间(术后冲洗时间、留管时间以及住院时间)明显短于TURP组(P<0.05),手术时间无统计学差异(P>0.05);术后随访6个月,两组IPSS、QOL、Qmax和PVR与术前比较均有明显改变(P<0.05),但两组改善程度无明显差异(P>0.05);TURP组并发症发生率明显高于TUPKRP组(P<0.05)。结论:TURP和TUP-KRP均是治疗良性前列腺增生症的有效方法,TUPKRP术后并发症发生率较低,患者恢复时间更短,安全性更高。Objective：To compare the feasibility and safety of transurethral bipolar vaporization of prostate （TUPKRP） and transurethral resection of the prostate （TURP） on benign prostatic hyperplasia. Methods.- A total of 87 patients with benign prostate hyperplasia were randomly divided into TUPKRP group （n=45） and TURP group （n=37） which sepeartely underwent TUPKRP and TURP methods. Blood loss volume, length of hospital stay, washing time, time needed indwelling catheter, IPSS, QOL, Qmax, PVR as well as incidence of complications were compared between the two groups. Results. TUP- KRP group showed significant less blood loss, shorter time needed indwelling catheter and hospital stay than the TURP group （P〈0.05）. But the two groups showed no significant difference in operation time （P〉0.05）. A 6 month follow up showed both groups showed significant improvements in IPSS.QOL. Qmaxand PVR after undergoing surgery （P〈0.05）, but no significant difference was observed between the two groups （P〉0.05）. TURP group showed significant higher incidence of complications comparing with TUPKRP group （P〈0.05）. Gonclusions： Both methods were therapy for benign prostatic hyperpla sia, but TUPKRP shows advantages of lower complication incidence, shorter recovery time and higher safety.
