国产羟乙基淀粉用于全麻诱导期容量治疗的临床研究

摘　　要：目的:评估国产6%羟乙基淀粉130/0.4注射液用于诱导期容量治疗的有效性。方法:全身麻醉手术患者90例,ASAⅠ～Ⅱ级,随机分为对照组(A组,n=30)、国产羟乙基淀粉组(B组,n=30)、进口羟乙基淀粉组(C组,n=30)。A组于全麻诱导开始时按传统速度15mL.kg-1.h-1输注乳酸钠林格氏液;B、C组于麻醉诱导前20min分别输入国产羟乙基淀粉和进口羟乙基淀粉6mL/kg,诱导开始加快输液速度,10min内输入。观察记录输液前即刻(T0)、诱导前即刻(T1)、诱导后插管前即刻(T2)、插管后5min(T3)、10min(T4)各组的收缩压(SBP)、舒张压(DBP)、心率(HR)和中心静脉压(CVP)。结果:麻醉诱导后各时段A组的SBP、DBP低于B、C组(P<0.05～0.01);3组CVP基础值正常偏低,B、C组输液后逐渐升高,但均在正常范围内,与A组各时点比较差异有统计学意义(P<0.01);B、C组间各时段各项指标差异均无统计学意义(P>0.05)。结论:国产羟乙基淀粉130/0.4用于诱导期容量治疗扩容效果安全可靠,与进口6%羟乙基淀粉130/0.4作用相似。Objective： To evaluate the efficacy and safety of domestic hydroxyethyl starch130/0.4 （Tianqingning） in the volume treatment during general anesthesia induction. Methods： Ninety patients with ASA Ⅰ- Ⅱ undergoing general anesthesia were randomly divided into 3 groups（A,B,C） with 30 in each group. In group A lactated Ringer＇s solution was infused at 15 mL· kg-1· h-1 as soon as induction of anesthesia started. In group B and C Tianqingning （B） or Voluven（C） 6 mL/kg was infused in 20 rain before induction of anesthesia. As soon as induction started another 6 mL/kg of Tianqingning or Voluven was infused in 10 rain. systolic blood pressure （SBP）, diastolic blood pressure（DBP）, heart rate （HR） and central venous pressure （CVP） were observed before transfusion （TO）, before induction （T1）, intubation after induction （T2）, at 5 min after intubation （T3） and at 10 rain after intubation （T4）. Results：SBP and DBP at 5 time points were significantly lower in group A than that in group B and C（P〈0.05-0. 01）. CVP in three groups were low but were all in normal range. It increased gradually after infusion in group B and C, and were significant different to that in group A at 5 points（P〈0.01）. There were no significant differences in all indexes among group B and C（P2〉0.05）. Oonclusion. Application of Tianqingning for volume therapy is as effective and safe as Voluven during general anesthesia induction.
