非瓣膜性房颤与血栓形成前状态的相关因素分析

摘　　要：目的探讨血浆高敏C反应蛋白(hsCRP)、B型利钠肽(BNP)、P-选择素(PS)、D-二聚体(D-D)、左心房直径(LAD)、射血分数(LVEF)与非瓣膜性房颤患者血栓形成前状态(PTS)的关系。方法按食道心脏彩色超声检查(TEE)检查结果将98例非瓣膜性房颤患者分为房颤血栓前状态组和房颤对照组,再将两组分别再分为哈萨克族和汉族两个亚组,检测血浆中hsCRP、BNP、PS、D-D的浓度,经胸超声心动图检测LAD、LVEF。结果哈萨克族和汉族房颤血栓前状态组hsCRP、BNP、PS、D-D浓度及LAD与房颤对照组差异有统计学意义。Logistic回归分析结果显示hsCRP、PS、LAD与哈萨克族非瓣膜性房颤血栓形成前状态独立相关(P<0.05),而BNP、D-D浓度、LVEF与哈萨克族非瓣膜性房颤血栓形成前状态无独立相关。结论非瓣膜性房颤患者hsCRP、PS浓度水平增高、LAD扩大是非瓣膜性房颤血栓形成前的独立危险因素。Objective To investigate the relationship of patients with nonvalvular atrial fibrillati on prothrombotic state with hsc-reactie protein,plasma B-type natriuretic peptide,D-dimers,P-selectin,left atrium diameter and Left ventricular ejection fraction.MethodsThe patients with nonvalvular atrial fibrillation were divided into atrial fibrillation thrombosis state group and control group atrial fibrillation by the,TEE examination results and then divided the patients into the Hazak and the Han nationality.To detection hs-CRP,BNP,PS,D-D and detection LAD,LVEF by Transthoracic echocardiography.ResultsThere was significant difference between AF thrombosis state group and AF control group of creactie protein,B-type natriuretic peptide,D-dimers,P-selectin and left atrium diamete.Logistic regretssion found c-reactive protein,P-selectin and left atrium diameter with nonvalvular atrial fibrillation thrombosis former state was independent related（P0.05）.But B-type natriuretic peptide,D-dimers,left ventricular ejection fraction with nonvalvular atrial fibrillation thrombosis former state no significant correlation.ConclusionIn patients with nonvalvular atrial fibrillation,hsCreactie protein,P-selectin and left atrium diameter are independent thrombosis former risk factors.
