摘  要:目的探讨自体骨髓单个核细胞与藻酸钙凝胶移植对激素性股骨头坏死的疗效及其促进成骨及血管化作用。方法选用成年新西兰雄兔75只,造模型后,随机分为A、B、C、D 4组。A组不给予任何处理;B组在数字减影型X射线机(DSA)下用12号穿刺针减压;C组在DSA导视下穿刺注入0.5 ml的5×106的骨髓单个核细胞;D组用0.5 ml的5×106骨髓单个核细胞与藻酸溶液(方法同B,C组)注入股骨头内。术后4周及8周各组兔先行MRI扫描,之后处死动物取股骨头行常规组织学染色及用扫描电镜观察超微结构。结果术后第8周,D组MRI示低信号区明显缩小;组织病理学及扫描电镜表现空骨陷窝减少,成骨细胞多,新骨形成;D组空骨陷窝阳性数和微血管密度与A、B和C组比较差异有统计学意义(P<0.05)。结论自体骨髓单个核细胞和藻酸钙凝胶移植对激素性股骨头坏死修复作用较为理想,并能够促进坏死区骨的再生和血管化。Objective To explore the therapeutic effect and osteogenesis and vascularization effect of autologous bone marrow mononuclear cells(BM-MNC) with calcium alginate gel in treating glucocorticoid-induced necrosis of the femoral head.MethodsSeventy five adult New Zealand male rabbits were randomly divided into four groups as A,B,C and D.Group A was untreated control;Group B were undergone the core decompression by using 12 G biopsy needle with the help of digital subtraction angiography(DSA);0.5 ml of 5×106 bone marrow mononuclear cells was injected into the femoral head with helping of DSA in Group C;0.5 ml of 5×106 bone marrow mononuclear cells with calcium alginate gel was injected into the femoral head(with the the same method as B and C) in Group D.4 and 8 weeks after the operation,all rabbits were undergone MRI scanning,and then killed and their bilateral femoral heads harvested for HE and scanning electron microscopy(SEM).ResultsEight weeks after the treatment,MRI showed an obvious minification of the low signal region,and histopathology and SEM revealed a decrease of empty lacunae,an increase of osteoblasts and new bone formation in group D,with significant differences from the other three groups in the positive rate of empty lacunae and the density of microvessels(P0.05).ConclusionThe therapeutic effects of autologous BM-MNC transplantation of alginate gel in the treatment of glucocorticoid-induced necrosis of the femoral head is more ideal,and can promote osteogenesis and vascularization in the necrotic area.
