消痰散结中医治疗方案用于胃癌术后临床观察

摘　　要：目的观察消痰散结中医治疗方案(金龙蛇颗粒+华蟾素注射液)用于胃癌术后的临床疗效、毒性反应及对生活质量的影响。方法 83例胃癌术后患者随机分为消痰组(消痰散结中医治疗方案)41例、化疗组(OFL化疗方案)42例,治疗3个月,评价2组患者肿瘤标志物变化、毒性反应及生活质量(卡氏评分与QLQ-30评分)。结果消痰组与化疗组CA19-9、CA12-5、CA72-4、CEA治疗前后差异无统计学意义(P>0.05)。毒性反应:白细胞降低、感觉性神经病、呕吐、厌食化疗组高于消痰组(P<0.01),静脉炎则反之(P<0.01)。生活质量化疗组低于消痰组,卡氏评分差异有统计学意义(P=0.006 1);QLQ-30评分:总体健康状况(P=0.003 2)、情绪功能(P=0.001 5)、疲倦症状(P=0.004 9)、恶心与呕吐症状(P=0.001 2)、食欲丧失症状(P=0.000 9)子量表差异有统计学意义。结论消痰散结中医治疗方案用于胃癌术后可减轻毒性反应,患者生活质量优于化疗。Objective To evaluate the efficacy,toxicity and the effect on quality of life（QOL） of TCM treatment of dispersing phlegm and eliminating stagnation in treating postoperative gastric carcinoma.Methods Eighty-three patients with postoperative gastric carcinoma were randomly divided into two groups：clearing phlegm group（n=41） received Jinlongshe granule combined with Huachansu injection,and control group（n=42） accepted chemotherapy（OFL protocol for 3 cycles）.The changes of CA19-9,CA12-5,CA72-4 and CEA,toxicity and QOL（assessed by KPS and QLQ-30） were compared between two groups.Results The difference of effect on CA19-9,CA12-5,CA72-4 and CEA between two groups was insignificant.The incidence rate of leukocytes,neuropathy,sensory,vomiting and anorexia in control group was higher than that in clearing phlegm group,but that of phlebitis was lower（P0.05）.The KPS had significant difference between two groups（P=0.006 1）.The QOL improved significantly in clearing phlegm group.The items of QLQ-30 including global health（P=0.003 2）,emotional functioning（P=0.001 5）,fatigue（P=0.004 9）,poor appetite（P=0.000 9）,nausea and vomiting（P=0.001 2） had significant difference between two groups.Conclusion TCM treatment of dispersing phlegm and eliminating stagnation has better efficiency for postoperative gastric carcinoma with tolerable toxicity and higher QOL.
