糖代谢紊乱与动脉振荡顺应性的相关性研究

摘  要:目的探讨动脉弹性指数(C1、C2)与Ⅱ型糖尿病的相关性。方法收集50例Ⅱ型糖尿病患者和50名健康对照者进行研究。采用美国CVProfilorTMDO-2020型动脉弹性功能测定仪系统测量血管弹性(C1、C2)。结果在年龄、性别、体重指数和腰臀比无明显差异时,2型糖尿病患者组大动脉弹性指数(C1)、小动脉弹性指数(C2)较正常对照者明显下降(P<0.05);多元逐步回归分析显示:C1与脉压、脉率、HOME-β、IR负相关,与ISI正相关,C2与收缩压负相关,与LDL-C、WHR正相关;Ⅱ型糖尿病组胰岛素敏感指数、胰岛β细胞功能较对照组明显下降(P<0.05),甘油三酯和LDL-C较对照组明显升高(P<0.05)HDL-C较对照组明显下降(P<0.05)。结论 2型糖尿病患者动脉弹性指数降低,脉压对C1影响较大。Objective To explore the relationship between arterial compliance and type 2 diabetes mellitus.Methods This study was conducted in 50 patients with type 2 diabetes mellitus and 50 normal controls.Large arterial compliance(C1) and small arterial compliance(C2) were measured by CVP rofilor TMDo─2020 cardiovascular profiling system.Meanwhile the plasma glucose,blood pressure,blood lipid,height and weight were measured,body mass index,waist hip ratio,insulin sensitivity index,insulin resistance and insulin β cell function were also calculated.Results Compared with patients of normal controls,C1 and C2 were significantly decreased in type 2 diabetes mellitus group.Multivariable regression analysis revealed C1 related negatively with pulse pressure,pulse rate,HOME-β,IR,positively with ISI,While C1 related negatively with systolic pressure,positively with LDL-C and WHR.The levels of insulin sensitivity index and insulinβ cell function were remarkably lower in type 2 diabetes mellitus,the levels of triglycerides and LDL-C were remarkably higher in type 2 diabetes mellitus.Conclusion Arterial compliance are decreased significantly in type 2 diabetes mellitus.Pulse pressure is the main determinant factor for C1.
