三七皂甙治疗糖尿病周围神经病变的meta分析

摘　　要：目的系统评价三七皂甙(PNS)治疗糖尿病周围神经病变(DPN)的疗效与安全性.方法采用Cochrane系统评价方法,计算机检索和手检相配合,查找相关数据库和期刊,纳人有关三七皂甙治疗糖尿病周围神经病变的随机对照试验,质量评价按照Cochrane系统评价员手册推荐的质量评价标准评价纳入研究质量,并对同质研究进行Meta分析.结果共纳入了7个RCT,包括592例DPN患者.Meta分析结果显示,PNS改善DPN症状及体征均优于维生素B12,其RR=1.68,95%CI为1.46%～1.93%.PNS对DPN神经传导速度的改善亦优于维生素B12.7个研究未见不良反应的报道.结论 PNS对治疗DPN可能有一定疗效,但由于尚缺乏高质量的RCT证据支持,PNS治疗DPN的疗效尚不能作出最后结论,还需进行更多高质量的RCT才能得出肯定性结论.Objective To evaluate the efficacy and safety of Sanchinoside for diabetic peripheral neuropathy（DPN）.Methods By using Cochrane systemic evaluation method,we retrieved related database and journals by computer retrieval and handiwork retrieval.The randomized controlled trials of clinical therapeutic studies on DPN treatment by Chinese medicine herb PNS were included and analyzed according to the criteria of the Cochrane Handbook.Meta analysis was used in this study.Results Seven RCTs involving 592 patients were included.Meta-analysis results indicated PNS was more effective than vitamin B12 on pain or numbness of extremities of DPN]RR =1.68,95%CI1.46%～1.93%.and motor nerve conduction velocity change of common peroneal nerves and median nerves.No one trial reported the side effects of PNS.Conclusions PNS infusion may have positive effect on DPN.However,the evidence is not strong enough due to the general low methodological quality,so we can＇t draw a reliable conclusion about the effects of PNS on DPN at the moment.Further large randomized,double blind,placebo-con-trolled trials are needed.
