多囊卵巢综合征体痤疮患者性激素水平变化及临床分析

摘　　要：目的分析多囊卵巢综合征（polycysticovarysyndrome，PCOS）患者雄激素水平与痤疮的关系以及炔雌醇环丙孕酮片治疗PCOS患者痤疮的效果。方法选取PCOS患者68例，分为痤疮组（36例）和非痤疮组（32例）两组，测定患者血清性激素，用炔雌醇环丙孕酮片治疗3个疗程或腹腔镜下卵巢多点打孔术后3个月复查血清性激素水平，同时选取正常健康体检者46例为对照组，对痤疮组患者治疗前后进行皮损定级评估。结果治疗前痤疮组血清睾酮的水平明显高于非痤疮组与对照组（P〈0．01）；非痤疮组睾酮水平亦明显高于对照组（P〈0．01）；痤疮组与非痤疮组患者血清LH及LH／FSH明显高于对照组（P〈0．01），3组间FSH的差异无统计学意义（P〉0．05）。治疗后痤疮组、非痤疮组患者雄激素均明显下降（P〈0．01）；PCOS痤疮组皮损疗效评价：治愈10例；显效13例；好转9例；无效4例；总有效率88．9％。结论PCOS患者表现痤疮与雄激素的升高有关，炔雌醇环丙孕酮片对此类痤疮皮损治疗有较好疗效。Objective To investigate sex hormone levels in polycystic ovarian syndrome （PCOS） patients with acne. Methods Sixty eight patients with PCOS were enrolled in the study including 36 cases with acne and 32 non-ache patients; while 46 healthy women served as controls. Serum sex hormones were determined before and 3 months after diane-35 treatment or laparoscopic ovarian drilling in patients. PCOS patients were graded before and after treatment for ache. Results Before treatment, the serum testosterone level of PCOS patients was significantly higher than that of controls （P〈 0.01）; and that of PCOS patients with ache was significantly higher than that of non-ache patients （P〈 0.01）. Serum LH and LH/FSH levels of PCOS patients were significantly higher than those of controls （P〈 0.01）, but there were no significant differences between ache patients and non-acne patients （P 〉 0.05）; there were no differences in FSH levels among 3 groups （P 〉 0.05）. After treatment, the serum testosterone levels of PCOS patients with acne and non-acne patients were significantly decreased （P〈 0.01）. After treatment 10 PCOS patients with acne were cured, 13 were markedly effective; 9 were improved; and 4 patients had no response with an overall effective rate of 88.9%. Conclusion The acne of PCOS patients is related to the increase of androgen levels, and diane-35 is an effective therapeutic agent.
