妊娠高血压综合征产妇剖宫产手术的麻醉体会

摘　　要：目的:探讨妊娠高血压综合征(简称妊高征)患者在剖宫产手术中的麻醉方法及效果。方法:回顾性分析2008年1月～2010年12月剖宫产结束分娩的135例妊娠高血压综合征患者的临床资料及麻醉处理情况,其中,87例采用连续硬膜外麻醉,48例采用气管内插管全身麻醉。结果:135例产妇麻醉过程平稳,手术顺利,连续硬膜外麻醉阻滞平面为T9～L2,其中,31例产妇可感知牵拉痛。气管内插管全身麻醉产妇未出现抽搐或烦躁等状况。135例产妇共娩出137例活产胎儿,死胎2例。产妇术后均治愈出院。结论:在妊高征产妇的麻醉过程中,需严密做好监测工作,出现异常情况及时处理,方能减少其并发症发生率及死亡率,保障母婴安全。Objective： To investigate the anesthesia methods and effects on the cesarean section parturients with pregnancy induced hypertension syndrome.Methods： The clinical data and the anesthesia methods of 135 parturients with pregnancy induced hypertension syndrome treated by cesarean section were analyzed retrospectively.87 cases of them were treated by continuous epidural anesthesia,48 cases were treated by endotracheal intubation general anesthesia.Results： The anesthesia process in all of the 135 cases were stationary,and the operative process was successful.The block levels of the parturients treated by continuous epidural anesthesia were T9-L2,31 cases felt the dragging pain.All of the parturients were treated by endotracheal intubation general anesthesia and convulsion or dysphoria were not occurred.137 live birth fetus were dilivery,2 dead.All the parturients were cured and left hospital.Conclusion： During the anesthesia process,the changes of condition,processes the abnormal situation immediately should be closely monitored,so that the complication rate and mortality rate can be reduced to ensure the mother-infant safety.
