摘  要:目的研究浓度对左旋布比卡因臂丛神经阻滞药代动力学的影响。方法 20例择期上肢手术患者,随机分为Ⅰ和Ⅱ组,分别予0.25%和0.375%左旋布比卡因(0.4 ml/kg)行肌间沟入路臂丛神经阻滞。分别于注药后10、20、30、45、60、90、120、180、240、360、480和720 min采静脉血2 ml,测定左旋布比卡因血药浓度,3P97软件计算药代动力学参数。结果两组主要药代动力学参数t_(1/2β)、T_(max)和C_(max)分别为(4.57±0.69,4.98±0.45)h、(0.26±0.12,0.45±0.26)h和(1.02±0.27,0.90±0.13)mg/L。结论相同剂量不同浓度左旋布比卡因臂丛神经阻滞,高容量组C_(max)较低容量组高。Objective To evaluate the effect of concentration to pharmaeokinetics of levobupivacaine for brachial plexus block. Methods 20 patients, undergoing elective upper shoulder surgery, were randomly derided into two groups, and were injected with levobupivacaine 0. 25% or 0. 375% (0.4 ml/kg) for intersealene brachial plexus block. Venous blood were collected for assay of the total plasma concentration of levobupivacaine at 10, 20, 30, 45, 60, 90, 120, 180, 240, 360, 480 and 840 rain after injection. The pharmacokinetic parameters were determined from plasma concentration - time data with 3P97 software package. Results The main pharmacokinetic parameter such as t1/2β, T=max , and Cmax was (4.57 ±0. 69, 4.98±0.45) h, (0.26±0.12,0.45 ±0.26) h, and (1.02±0.27;0.90±0.13) rag/L, respectively. Conclusion The larger volume group produces higher Cmax than the lower volume group when the same dosage levobupivaeaine is injected.
