摘  要:目的探讨导致内固定断裂松动原因。方法总结2002年1月至2008年11月应用后路内固定系统治疗的316例脊柱骨折患者的临床资料,其中26例术后内固定失败或行二次在我院翻修手术患者临床资料应用Logistic回归分析比较两组患者的性别、年龄、植骨质量、固定范围,支具保护,手术技术应用等与脊柱内固定失败的相关强度,推测可能脊柱内固定失败的危险因素。结果植骨质量、支具保护、手术技术应用等3个因素(P<0.05)是内固定失败原因的危险因素。结论脊柱内固定失败是多种因素作用所致,从翻修的26例患者脊柱内固定手术评估时发现,植骨质量、支具保护、手术技术应用这几个因素是胸腰段后路手术内固定系统断裂松动原因的危险因素。Objective To analyze the risk factors of the reasons for failure of posterior pedicle screw internal fixation in thoraeolumbar fractures. Methods From January 2002 to November 2008, a total of 316 patients had been operated in our hospital, in which 26 patients failedposterior pedicle screw internal fixation after thoraeolumbar fracture. Patient's sex, age, bone graft quality, the procedure of internal fixation, external fixation, procedure approaches were analyzed and compared by Logistic Regression analysis. The possible risk factors of the failure were analyzed. Results The risk factors of bone graft quality, external fixation, operation techniques were statistically significantly different between those who had failed posterior pedicle screw internal fixation after thoracolumbar fractures than those who had not failed. Conclusion The failure of posterior pedicle screw internal fixation after thoracolumbar fractures is caused by many reasons. We should pay more attention to the risk factors of bone graft quality, external fixation and procedure approach.
