交锁髓内钉远端锁钉盲置法的可行性

摘　　要：目的探讨交锁髓内钉远端锁钉盲置法的可行性。方法应用交锁髓内钉治疗下肢骨折57例,远端锁钉均采用盲置法置入,直视下可以看到髓内钉主孔后拧入锁钉。结果随访6-15个月,57例患者均骨折愈合。其中,延迟愈合8例,改动力性固定3个月后愈合;远、近端锁钉100%在位,无锁钉断裂和退出。结论交锁髓内钉远端锁钉盲置法简单可行,锁钉在位率高,可同时减少医源性X线辐射。Objective To investigate the feasibility of blind implantation of distal locking nail for interlocking intramedullary nail.Methods The distal locking nail was implanted using blind implantation method in 57 patients with lower limb fracture treated with interlocking intramedullary nail.The locking nail was screwed when the main hole of the intramedullary nail was seen under direct vision.Results The patients were followed up for 6 to 15 months,during which fratured bone healing occurred in all cases,in whom 8 cases delayed union and got fracture healing in 3 months after the dynamization.All of the proximal and distal locking nails were eutopic without any case of locking nail fracture and exit.Conclusion The blind implantation method is simple,feasible and minimally invasive with the advantages of reducing iatrogenic X-ray radiation and increasing eutopic rate of interlocking intramedullary nail implantation.
