血清白细胞介素-6、白细胞介素-10及肿瘤坏死因子-α浓度与脓毒症患儿预后的相关性研究

摘　　要：目的探讨脓毒症患儿血清白细胞介素（interleukin，IL）-6、IL-10及肿瘤坏死因子（tumor necrosis factor，TNZ）-仅的浓度与脓毒症预后的相关性。方法对象为PICU收治的57例脓毒症患儿，其中存活44例（脓毒症存活组），死亡13例（脓毒症死亡组）。另有65例非脓毒症患儿为对照（非脓毒症组）。分别比较各组患儿危重症评分、脏器损害数目及血清中IL-6、IL-10、TNF-α、IL-6／1L．10、TNF-α/IL-10的水平。结果与非脓毒症组及脓毒症存活组比较，脓毒症死亡组患儿脏器损害数目多，危重症评分低（P〈0．05）；血清炎症细胞因子IL-6的表达增加（P〈0．05），血清抗炎症细胞因子IL-10水平明显升高（P〈0．05），TNF-α/IL-10比值明显降低（P〈0．05），但脓毒症存活组与非脓毒症组比较差异无统计学意义（P〉0．05）。3组血清TNF-α浓度及IL-6／IL-10比值差异无统计学意义（P〉0．05）。结论脓毒症患儿IL-6、IL-10浓度越高及TNF-α/IL-10比值越小，预后越差。Objective To investigate the correlation of serum concentrations of interleukin （IL）,6, IL-10 and tumor necrosis factor（TNF） -α and the prognosis of sepsis in children. Methods Fifty-seven septic patients in PICU were enrolled in the study, of whom 44 patients survived（ sepsis survival group）, 13 patients died（ sepsis dead group）. Another 65 cases of non-sepsis patients served as control （non-sepsis group）. The critical illness score, the number of damaged organs, and serum levels of IL-6, IL-10, TNF-α, IL-6/IL-10, TNF-α IL-10 in each group were compared. Results The critical illness score was decreased and the num- ber of damaged organs was more in sepsis dead group than those in non-sepsis group and sepsis survival group. Compared with non-sepsis group and sepsis survival group, the expression of IL-6 and IL-10 increased,TFN-α/IL-10 ratio significantly decreased in sepsis dead group （P 〈 0. 05 ）, but the difference between the sepsis survival group and non-sepsis group was not statistically significant （P 〉 0. 05 ）. The serum levels of TNF-α and IL-6/IL-10 ratio showed no significant difference among three groups （ P 〉 0. 05 ）. Conclusion In sepsis patients,the serum concentrations of IL-6 ,IL-10 are higher and the TNF-α/IL-10 ratio is smaller, the prognosis is worse.
