左炔诺孕酮宫内缓释系统用于治疗无排卵性功血的效果观察

摘　　要：目的:观察和比较左炔诺孕酮宫内缓释系统(LNG-IUS)和口服炔诺酮治疗无排卵性功血的疗效。方法:将65例无排卵性功血患者随机分为LNG-IUS组(30例)和炔诺酮组(35例)进行治疗,对治疗前及治疗后3个月、6个月的月经情况、经前子宫内膜厚度、血红蛋白、激素水平、肝肾功能、血脂及治疗满意度等进行比较。结果:两组治疗6个月后月经量均明显减少,血红蛋白显著增加,经前子宫内膜变薄,肝肾功能及血脂水平均无明显变化。与治疗前相比,LNG-IUS组激素水平无明显变化,炔诺酮组雌二醇水平下降,孕酮水平升高;LNG-IUS组治疗满意度高于炔诺酮组。结论:LNG-IUS和炔诺酮治疗无排卵性功血均效果明显,但LNG-IUS较炔诺酮治疗更具优势。To evaluate and compare the effects of levonorgestrel - releasing intrauterine system （ LNG - IUS） and oral norethindrone on the treatment of anovulatory dysfunctional uterine bleeding. Methods ： Sixty - five Patients with anovulatory dysfunctional uterine bleeding were randomly divided into LNG - IUS group （ n = 30） and the norethindrone group （ n = 35）. Menstruation, endometrial thickness, serum concentrations of hemoglobin and sex hormone, liver and kidney function, blood lipids and treatment satisfaction were evaluated before and after treatment for 3 and 6 months. Results ： After 6 months of treatment, the amount of menses significantly reduced, and the concentration of hemoglobin increased significantly, and endometrium became significantly thinner. The liver, kidney function and lipid levels did not change in both groups. Levels of sex hormones did not change significantly in LNG - IUS group, meanwhile, serum estradiol level reduced in the norethindrone group. Conclusion ： Both LNG - IUS and oral norethindrone have obvious effects on the treatment of anovulatory dysfunctional uterine bleeding, while the former could be better.
