心房复极波增大诱发房早与阵发房颤的关系

摘　　要：目的探讨心房复极波增大诱发的房性早搏与阵发性心房颤动的关系。方法对频发房早患者进行随访,行心电图和动态心电图检查,房早前有心房复极波增大的为观察组,反之为对照组。观察两组阵发房颤的发生率、持续时间、发作时的临床诊断和并发症。结果与对照组比较,观察组阵发房颤的发病率较高、发作阵次较多、持续时间较长,分别为31.13%vs.18.92%、457vs.376阵、(1967.34±1372)vs(.1251.28±1715.97)s;观察组患心肺脑疾病、高血压病、内分泌及代谢性疾病较多,且多种疾病共存,差异均有显著意义;而发作阵发性心房颤动时的并发症差异无显著意义。结论心房复极波增大预示心房电不稳定,与心房颤动的发生有密切关系。Objective To explore the relationship between the premature atrial contraction caused by the amplification of the atrial repolarization and paroxysmal atrial fibrillation. Methods To examine patients with premature atrial contraction by electrocardiogram or dynamic electrocardiogram. We have those patients with atrial repolarization wave amplification before premature atrial contraction are in the observation group, and those without atrial repolarization wave are in the compared group. We observed the incidence, frequency, duration, clinical diagnosis and complications in the two groups. Results The observation group’s paroxysmal atrial fibrillation incidence and the frequency are higher, the duration is longer, compared to the other group. The patients are suffering from heart, lungs and brain diseases or hypertension, the endocrine disease, metabolic disease in the observation group. The differences between data of the two groups are significant. The differences of the complications when paroxysmal atrial fibrillation breaks out are not significant. Conclusion The amplification of the atrial repolarization wave indicates the unstablity of atrial electric activity and the intimate connection between the amplification of the atrial repolarization and paroxysmal atrial fibrillation. We should pay attention to the effect of amplification of the atrial repolarization in the electrocardiogram.
