摘  要:目的建立肛提肌力学模型,分析病理状态下肛提肌的应力分布,为从力学角度认识女性盆底功能障碍性疾病(PFD)提供参考。方法基于磁共振(MRI)影像学资料,采用MIMICS三维重建软件重构肛提肌的几何结构,采用有限元分析方法仿真计算肛提肌的应力分布。结果盆腔脏器脱垂和不同程度的压力性尿失禁,在肛提肌中最大应力出现位置相同,数值不同,盆腔脏器脱垂和Ⅰ型压力性尿失禁肛提肌中的最大应力偏高,Ⅲ型压力性尿失禁的最低。结论盆腔脏器脱垂和不同程度的压力性尿失禁四种病理状态下的高腹压,可对肛提肌产生较大的应力,容易造成肛提肌组织损伤,这个结果可为临床预防PFD提供一定的理论参考。Objective To build the mechanical model of the m. levator ant to analyze its stress distrihution in pathological state, and provide reference to further know pelvic floor dysfunetion(PFD)in terms of mechanics. Methods Based on magnetic resonance imaging (MRI) of the pelvic floor, to reconstruct the geometric structure of the m. levator ani using the medical imaging software MIMICS and to simulate the stress distribution of the m. levator ant using the finite element method. Results For both the viscera organs prolapse and the stress urinary incontinence the maximum stress position were same but the magnitude was difference in the m. levator ani. The maximum stress in the m. levator ani was more higher between the viscera organs prolapse and the type Ⅰ stress urinary incontinence, but it was lowest for the type Ⅲ stress urinary incontinence among type Ⅰ to Ⅲ stress urinary incontinence and the viscera organs prolapse. Conclusion High intra-abdominal pressure will all produce high stress in the In. levator ani for the four pathological states, thus it is easy to damage the m. levator ani. The result will provide some theory rcference for preventing PFD in clinic.
