不同分期糖尿病肾病患者血清IL-18、CRP水平的变化及意义

摘　　要：目的探讨不同分期糖尿病肾病(DN)患者血清IL-18、C反应蛋白(CRP)水平的变化及意义。方法选择97例2型糖尿病患者,根据尿白蛋白排泄率(UAER)分为3组:正常白蛋白尿组35例、微量白蛋白尿组30例和大量白蛋白尿组32例,另选健康体检者35例作为对照组。采用ELISA法检测各组患者血清IL-18、CRP水平,并对其影响因素进行分析。结果 2型糖尿病患者血清IL-18、CRP水平明显高于对照组(P均<0.05),且随着UAER的增加,IL-18水平逐渐增高(P均<0.01)。Pearson相关分析显示,血清CRP、IL-18均与病程、收缩压、糖化血红蛋白、空腹血糖、TG、TC、LDL-C、尿素氮、血肌酐、UAER呈正相关(rIL-18分别为0.766、0.628、0.568、0.388、0.375、0.400、0.512、0.516、0.698、0.792;rCRP分别为0.777、0.701、0.416、0.327、0.308、0.307、0.449、0.611、0.817、0.893,P均<0.01),与HDL-C呈负相关(rIL-18=-0.652、rCRP=-0.546,P均<0.01),且两者互呈正相关(r=0.890,P<0.01)。结论血清IL-18、CRP与DN的发生、发展有关,两者联合检测有助于DN的早期诊断。Objective To study the serum IL-18,C-reactive protein（CRP） level changes and their significances in diabetic nephropathies patients in different stages.Methods Ninety-seven cases of type 2 diabetes were chosed,and they were divided into three groups,namely normal albuminuria group（n=35）,microalbuminuria group（n=30） and proteinuria group（n=32）,according to urinary albumin excretion rate（UAER）.Thirty-five healthy individuals were selected as the control group.The levels of serum IL-18 and CRP were detected by ELISA,and their influential factors were analyzed.Results Compared with control group,IL-18,CRP levels in diabetic patients were significantly higher（P〈0.05）,and IL-18 levels gradually increased with the increasing of UAER（P〈0.01）.Pearson correlation analysis showed that serum CRP,IL-18 were positively associated with disease,systolic blood pressure,glycated hemoglobin,fasting glucose,TG,TC,LDL-C,blood urea nitrogen,serum creatinine,UAER（rIL-18=0.766,0.628,0.568,0.388,0.375,0.400,0.512,0.516,0.698,0.792;rCRP=0.777,0.701,0.416,0.327,0.308,0.307,0.449,0.611,0.817,0.893,P〈0.01）,and negatively correlated with HDL-C（rIL-18=-0.652,rCRP=-0.546,P〈0.01）.There was a positive correlation between IL-18 and CRP（r=0.890,P〈0.01）.Conclusions IL-18 and CRP may be involved in the development of type 2 diabetic nephropathies,combined detection of both have important role in early diagnosis.
