摘  要:目的探讨同期行经尿道前列腺电切术和无张力疝修补术治疗前列腺增生并发腹股沟疝的效果。方法对该院2005年7月~2011年2月采用经尿道前列腺电切术同期无张力疝修补术治疗的38例前列腺增生症并腹股沟疝患者临床资料进行回顾性分析。本组38例,平均年龄69(55~86)岁,均为斜疝,其中3例为复发性斜疝,合并高血压7例,2型糖尿病3例,慢性支气管炎3例;先行患侧腹股沟疝无张力修补术,疝修补方法参照中华医学会外科学分会疝和腹壁外科学组推荐的方法;疝修补术完成后再改截石体位,采用Wolf或Circon双鞘电切镜,行经尿道前列腺电切术。结果本组38例均顺利完成手术,无张力疝修补术时间30~95 min,平均45 min,经尿道前列腺手术时间35~125 min,平均55 min,切除前列腺重量15~60 g,平均25 g,出血量40~300 ml,平均90 ml,均未输血。结论采用同期经尿道前列腺电切术和无张力疝修补术治疗前列腺增生并发腹股沟疝,避免了患者二次手术的痛苦,具有损伤小,并发症少等优点,疗效满意。Aim To investigate the surgical effects of transurethral resection of prostate in combination with tension-free hemioplasty in patients with benign prostate hyperplasia(BPH)and inguinal hernia.Methods The clinical material carries on the review analysis that 38 cases of BPH complicated with inguinal hernia were treated with TURP and one stage tension-free repair of inguinal hernia from July 2005 to February 2011.In this group 38 cases,with an the average age of 69(55~86) year old,were slanting hernia,3 recurrence slanting hernia,merge the hypertension 7,2 Diabetes 3,chronic bronchitis 3.Results They all went through surgical operation smoothly,with tension-free Hernioplasty time of 30~95 min,average 45 min,transurethral Resection of Prostate time 35~125 min,average 55 min,the excision prostate gland weight 15~60 g,average 25 g,bleed measure a 40~300 ml,average 90 ml.Nobody had blood tranfusion.Conclusion Transurethral resection of prostate in combination with tension-free hernioplasty has a satisfied effect for benign prostate hyperplasia with inguinal hernia,has avoided second-time surgery,has the advantages of small damage and few complications.
