精子动力学参数在常规IVF中的应用初探

摘　　要：目的探讨各动力学参数对常规体外受精（IVF）受精率的影响。方法分别对103例常规IVF周期病人的新鲜精液、优化后及孵育后3个阶段的精液进行计算机辅助的精子分析（CASA）的动力学参数检测，同时根据第五版WHO《人类精液检验及处理实验室手册》，对各组间正常和异常精液进行动力学参数的差异分析。结果（1）异常精液组在新鲜精液的大部分参数水平显著低于正常精液组，但经过优化处理以及一定条件下的孵育后，这种差异消失；（2）检测的大部分参数在新鲜精液和优化后精液之间均有极显著性差异，在孵育后精液和优化后精液之间均有显著性差异。此外，各参数同受精率之间均无显著相关性（3）受精组和不受精组间的各动力学参数无显著性差异。结论使用精液优化方法以及加精之前的孵育过程可以显著改善精子的活动力和直线运动能力；CASA分析各动力学参数的结果，可作为病人精液分析结果诊断的依据，但不能以此推断IVF结局。Objective To investigate the effects of sperm motility parameters on the outcome of fertilization. Methods The sperm motility parameters for the fresh semen samples, purified semen samples, and samples after incubation from 103 IVF cycles were detected respectively by computer assisted sperm analysis （CASA）. The differences in sperm motility parameters between normal and abnormal semen groups were statistically analyzed according to the 5th WHO guide. Results （1） There were significant differences in many sperm motility parameters between normal and abnormal groups, and these differences were changed after purification and incubation. （2） Many sperm motility parameters had extremely significant differences between fresh semen samples group and purified samples group, and had significant differences between purified samples and samples before insemination. And, there was no significant correlation between the sperm motility parameters and fertilization rate in vitro. （3） There were no significant differences in sperm motility parameters between fertilization and non-fertilization groups. Conclusion Semen motility and linear motion ability can be significantly improved after purification and incubation. Sperm motility parameters can be used as basis of diagnosis, but not as prediction of of IVF result.
