腋中线双孔法VATS肺大泡切除术治疗中青年自发性气胸的应用研究

摘　　要：目的探讨腋中线双孔法VATS肺大泡切除术治疗中青年自发性气胸的应用价值。方法选30例中青年患者随机按手术方式分为腋中线双孔法实验组(n=15)和传统三孔法对照组(n=15),疗效评价指标包括手术时间、术中出血量、术后使用杜冷丁量、术后住院日、切口总长度、切口美观程度及术后复发率。结果两组手术时间、术后复发率及术后住院日差异均无统计学意义(P>0.05);与对照组比较,实验组术中出血量少、术后使用杜冷丁量少、切口总长度短及切口美观程度高,且差异均具有统计学意义(P<0.05)。结论腋中线双孔法与传统三孔法VATS肺大泡切除术治疗中青年自发性气胸疗效相同,但腋中线双孔法术后患者疼痛更轻、切口更为美观,其创伤更为轻微。Objective To investigate the effect of resection of pulmonary bullae with two-port VATS technique on the midaxillary line for spontaneous pheumthorax of young and middle-aged patients.Methods A total of 30 patients were divided into test group（two-port VATS technique on the midaxillary line n=15） and control group（traditional three-port VATS technique n=15）.The operative time,volume of operative haemorrhage,volume of postoperative dolantin,postoperative hospital stay,total length of operative cut,aesthetic measure of operative cut and rate of recurrece were compared between the two groups.Results No significant difference existed in the operative time,rate of recurrence and postoperative hospital stay between the two groups（P0.05）.The volume of operative haemorrhage,volume of postoperative dolantin,total length of operative cut of test group were significantly less than those in the control group（P0.05）.The aesthetic measure of operative cut of test group was significantly better than that in the control group（P0.05）.Conclusion The effect of resection of pulmonary bullae with two-port VATS technique on the midaxillary line for spontaneous pheumthorax of young and middle-aged patients is as equal as that of traditional three-port VATS technique.Whereas it can alleviate the operative pain,improve the aesthetic measure of operative cut,and significantly reduce the trauma.
