封闭式负压引流联合湿性敷料治疗Ⅳ期压疮的效果观察

摘　　要：目的探讨封闭式负压引流(vacuum assisted closure,VAC)联合湿性敷料治疗较大面积Ⅳ期压疮的疗效。方法按入院时间顺序随机将66例78处Ⅳ期压疮患者分为对照组32例36处压疮,采用湿性敷料法;实验组34例42处压疮,采用VAC联合湿性敷料法。比较两组患者清创完成时间、4周材料费用、换药时间及第10周治疗效果。结果与对照组比较,实验组清创完成时间明显缩短,4周材料费用明显减少,但每次换药时间明显延长,两组比较,差异具有统计学意义(均P<0.05)。结论 VAC联合湿性敷料处理大面积Ⅳ期压疮较单独采用湿性敷料法治疗效果好,花费更少,值得临床推广应用。Objective To study the effects of vacuum assisted closure drainage（VAC） combined with synthetic dressing on stage Ⅳ pressure ulcers. Methods 78 cases with stage Ⅳ pressure ulcer were randomly divided into two groups. The control group（n = 36） was treated by synthetic dressing and the experiment group（n = 42） by VAC combined with synthetic dressing. The two groups were compared in terms of time for completing would cleaning,expense on 4 weeks material consumptions,time for changing the dressing and the curative effect at week 10. Results Compared with the control group,the time for wound cleaning in experiment group was significantly shortened,the average expense on 4 weeks material consumptions was significantly lessened but the time for changing the dressing was extended（all P 0.05） . Conclusion VAC combined with synthetic dressing is more effective and cheaper than synthetic dressing alone in the treatment of stage IV pressure ulcers.
