妇女假丝酵母菌属及滴虫性阴道炎患者支原体属合并感染的探讨

摘　　要：目的分别了解妇女假丝酵母菌性阴道炎、滴虫性阴道炎患者解脲脲支原体(Uu)、人支原体(Mh)、生殖支原体(Mg)、肺炎支原体(MP)的合并感染状况。方法应用套式PCR技术,对150例假丝酵母菌性阴道炎和74例滴虫性阴道炎患者的阴道拭子标本进行Uu、Mh、Mg、MP检测;部分病例进行套式PCR产物的直接全自动DNA测序。结果假丝酵母菌和滴虫性阴道炎患者均存在Uu、Mh、Mg和MP的合并感染;假丝酵母菌性阴道炎者Uu和Mh的合并感染率为66.7%和30.7%;滴虫性阴道炎者则为67.6%和48.6%;Uu、Mh、Mg和MP各1例临床标本的套式PCR产物经全自动DNA测序与标准株完全一致。结论妇女假丝酵母菌性阴道炎和滴虫性阴道炎患者均存在Uu、Mh、Mg和MP的合并感染,其中以Uu、Mh更为常见;建议临床医师对于合并支原体属感染者在抗真菌、抗滴虫治疗的同时应追加抗菌药物。OBJECTIVE To investigate the infection state of Ureaplasma urealyticum（Uu）,Mycoplasma hominis（Mh）,Mycoplasma genitalium（Mg） and Mycoplasma pneumoniae（MP） in the female vulvovaginitis patients infected with Candida and Trichomonas.METHODS Vulvae swab specimens from 150 monilial vaginitis patients and 74 trichomonas vaginitis patients were collected and detected by nested polymerase chain reaction（nPCR） to identify the existence of pathogens as Uu,Mh,Mg and MP.The nucleic acid was sequenced by the automatic DNA sequencer to some positive cases.RESULTS The combination infection of 4 kinds mycoplasma were found in the female vulvovaginitis patients infected with Candida and Trichomonas.In the monilial vaginitis patients,the merging infectious positive rates were 66.7%（Uu） and 30.7%（Mh）,respectively.In the trichomonas vaginitis patients,the merging infectious positive rates were 67.6%（Uu） and 48.6%（Mh）,respectively.The PCR products of Uu,Mh,Mg and MP were sequeced,the results showed that the fragments were uniform with standard sequences.CONCLUSIONS Both the merging infection of 4 Mycoplasmas exist in the female vulvovaginitis patients infected with Candida and Trichomonas.Uu and Mh are the most common Mycoplasma in these patients.It suggests that clinical doctor should add antibiotics therapy in the patents merged with Mycoplasma.
