Ⅰ类手术围术期抗菌药物应用的干预分析

摘　　要：目的通过干预措施促进医院Ⅰ类手术围术期抗菌药物的合理应用。方法通过对外科临床医师进行宣传教育,制定Ⅰ类手术围术期抗菌药物应用实施细则,对医院Ⅰ类手术围术期抗菌药物应用进行干预,随机选取干预前后病例(干预前105例,干预后116例)进行调查分析。结果干预前后所有病例均使用了抗菌药物,用药率100.00%;干预前后术前30min～2h预防用药率、术后二联用药率差异有统计学意义(P<0.05),干预后术前预防用药率提高49.88%,二联用药率下降13.36%,无≥三联用药病例;干预前后术后用药时间在<48、48～72、>72h方面差异无统计学意义;干预前后用药种类均以头孢菌素类居首。结论通过干预,医院Ⅰ类手术围术期抗菌药物应用仍存在许多不足之处,规范围术期抗菌药物的应用需多部门参与,共同提高抗菌药物的合理应用。OBJECTIVE To promote the rationality of the antimicrobial perioperative of Ⅰtypes surgery in our hospital through the intervention measures.METHODS Through to the surgery clinical doctors do propaganda education,Make Ⅰtypes of surgery perioperative detailed rules for the implementation of the antimicrobial application,In ourⅠtypes of surgery perioperative antimicrobial application to intervene,the cases with randomly selected before and after intervention（before the intervention was105 cases,after the intervention was116 cases） were investigated.RESULTS All the cases before and after intervention using antibiotics,the usage rate was 100.00%.Before and after intervention 0.5-2 h preoperative and postoperative prevention drug use ratio 2 league drug use proportion statistically significant（P〈0.05）,after intervention to prevent drug use preoperative higher 49.88%,two league drug use ratio fell 13.36%,No drug cases was triple.Before and after the treatment duration after intervention in 〈48 h,48-72 h,〉 72 h aspects is no statistical difference（P〉0.05）.Before and after intervention medicine species were cephalosporin preeminence.CONCLUSION Through the intervention measures ourⅠtypes of surgery perioperative antimicrobial application still have many shortcomings,standard perioperative period in the application of antimicrobial agents should be more participation in the department,and improve the application level of antimicrobial reasonable shoulder heavy responsibilities.
