抗菌药物临床应用连续性横断面调查

摘　　要：目的了解该院抗菌药物临床应用现况,为指导该院抗菌药物的合理应用和监测提供参考依据。方法在2008年1～12月每月下旬进行一次医院感染现患率调查的同时,进行抗菌药物使用情况的调查,并对结果进行统计分析。结果抗菌药物横断面平均使用率为29.1%,其中,重症监护室(IC U)、外科、五官科、儿科其使用率>50.0%,肿瘤科、内科、妇产科使用率<50.0%;预防性用药占42.6%,治疗性用药57.4%,联合性用药15.2%;其中手术科室以预防性用药为主,非手术科室以治疗性用药为主;细菌培养送检率为15.6%,检出的病源菌多为革兰氏阴性杆菌;抗菌药使用种类以头孢类为最多,其他依次为西林类、喹诺酮类、克林霉素类、氨基糖苷类、大环内酯和抗真菌类。结论抗菌药物合理应用的管理,需要综合各方面因素进行监测,并在实践中加以研究和总结才能有效。【Objective】 An antibiotic application investigation was carried out to provide the reference standard of justifiable antibiotic application and monitoring in our hospital.【Methods】 The current infection rates was investigated at the latter part of each month from January to December 2008.In the mean time,a survey of antibiotic application in hospital was also performed.The survey results were analyzed statistically.【Results】 The average usage rate of antibiotic was 29.1%.The rate was over 50% in departments of surgery,ENT and pediatrics,while in departments of oncology,medicine,obstetrics and gynecology,it was less than 50%.The rate of antibiotic usage for prevention,therapy and combination were 42.6%,57.4% and 15.2%,respectively.The antibiotic drugs were mainly used for prevention in surgery department,and for therapy in non-surgery department.The delivered rate of germiculture was 15.6%.Gram-negative bacilli were mostly found.The top category of antibiotic usage was cephalosporins,followed by penicillin,carbostyril,clindamycin,large ring lactone and antimycotic.【Conclusions】 The management of antibiotic application needs to be monitored from all aspects,and is becoming to be an effective approach through studying and summarizing in practice.
