摘 要:目的探讨定量组织速度成像技术(quantitative tissue velocity imaging,QTVI)评价慢性肺心病患者右室功能的临床价值。方法取52例慢性肺心病患者和60例正常人进行经胸超声检查,采用标准心尖四腔心切面,应用QTVI对右室侧壁中间部、后室间隔中间部、三尖瓣前叶及隔叶瓣环等部位进行定量分析。结果与正常对照组相比,慢性肺心病组三尖瓣前叶及隔叶瓣环处VS、VE和VE/VA均明显减低,差异具有显著性(P<0.05),VA在侧壁及后间隔的中间部有所升高,但在瓣环处均减低。慢性肺心病组瓣环处及中间部VS、VE和VE/VA在侧壁与后间隔比较无明显差异(P>0.05)。结论 Q TVI技术能准确、直观地反映肺心病患者右室收缩及舒张功能,具有重要的临床价值。【Objective】 To evaluate the clinical value of quantitative tissue velocity imaging(QTVI)in detecting right ventricular function of chronic cardiopulmonary disease.【Methods】 52 patients with chronic cardiopulmonary disease and 60 normal controls were studied with QTVI.QTVI indices of intermedius in right ventricle lateral wall and post-interventricular septum,tricuspid valve annulus were detected,and then compared with those of the control group.The indices included systolic peak velocity(VS),early-diastolic peak velocity(VE),late-diastolic peak velocity(VA),ratio of VE and VA(VE/VA).【Results】 Compared with normal controls,VS,VE and VE/VA were decreased significantly in tricuspid valve annulus of chronic cardiopulmonary disease(P 0.05).VA increased slightly in intermedius of right ventricle wall but decreased in tricuspid valve annulus.VS,VE and VE/VA were not significantly different in right ventricle lateral wall and post-interventricular septum of chronic cardiopulmonary disease(P 0.05).【Conclusions】 It has important clinical value,for QTVI in quantitating tricuspid valve annulus motion can exactly,directly assess the right ventricular function of chronic cardiopulmonary disease.
